Measuring Treatment Outcomes in Comorbid Insomnia and Fibromyalgia: Concordance of Subjective and Objective Assessments

Abstract: Study Objectives: In insomnia, actigraphy tends to underestimate wake time compared to diaries and PSG. When chronic pain co-occurs with insomnia, sleep may be more fragmented, including more movement and arousals. However, individuals may not be consciously aware of these arousals. We examined the baseline concordance of diaries, actigraphy, and PSG as well as the ability of each assessment method to detect changes in sleep following cognitive behavioral therapy for insomnia (CBT-I). Methods: Adults with inso… Show more

“…Because insomnia is diagnosed using self-reported indicators of poor sleep quality, self-reported measures are considered to be the most adequate measures of intervention effect in adults. 55,56 However, an objective (actigraph and/or EEG) measure of sleep should be used as a complement to self-reported measures 57 because such measures do not completely overlap. 51 Daytime assessments, such as self-reported sleepiness, could also provide complementary information on sleep quality because this measure is a sensitive indicator of insufficient sleep (as measured by EEG).…”

“…A meta-analysis of 15 studies 15,16,18,[20][21][22]24,27,33,[39][40][41]43,46,56 compared actigraphy to PSG for the assessment of TST in patients with suspected or diagnosed insomnia. See supplemental material, Figure S1b.…”

“…[2][3][4][5] Studies performed in recent years have supported the influence of central mechanisms in fibromyalgia. 6,7 The implication of central mechanisms in the etiopathogenesis of FMS suggests that FMS may be associated with neuropathic pain, and in this context, pain processing appears to be the leading central mechanism. Increased sensitivity to pain in patients with FMS has recently been more commonly associated with changes in the central nervous system and central sensitization.…”
