BackgroundIron deficiency (ID) frequently complicates inflammatory-mediated chronic disorders, irrespective of anaemia. Psoriasis is a chronic, immune-mediated skin disease with systemic pro-inflammatory activation; thus, these patients may be prone to develop ID. ID adversely affects immune cells function, which can further contribute to disease progression. This study investigates iron status in psoriasis.MethodsSerum concentrations of ferritin, transferrin saturation (Tsat), soluble transferrin receptor (sTfR), and hepcidin were assessed as the biomarkers of iron status in 39 patients with psoriasis (17 men, age: 47 ± 10 years) and 44 healthy subjects (30 men, age: 53 ± 6 years).ResultsCompared with healthy controls, patients with psoriasis demonstrated similar haematologic status but deranged iron status as evidenced by decreased Tsat and elevated sTfR (negative tissue iron balance) and low levels of hepcidin (depleted iron stores) (all P < 0.05 vs. controls). In patients, the levels of interleukin-6 (level of pro-inflammatory activation) significantly correlated with hepcidin (R = 0.54), but not with ferritin, Tsat, and sTfR. Biomarkers reflecting ID were not associated with the severity of the disease (assessed with the Psoriasis Area and Severity Index) but significantly correlated low body mass index (BMI). Patients with BMI < 24 kg/m2 compared with those with BMI ≥ 24 kg/m2 demonstrated lower levels of ferritin (40 ± 30 vs. 186 ± 128 ng/mL, P < 0.001) and hepcidin (4.9 ± 2.3 vs. 10.7 ± 6.7 ng/mL, P = 0.03).ConclusionPsoriasis is associated with deranged iron status characterized by depleted iron stores with concomitant unmet cellular iron requirements. The magnitude of these abnormalities is particularly strong in patients with low body mass index. Whether iron deficiency may become a therapeutic target in psoriasis needs to be investigated.

Epidemiological studies suggest an increased prevalence of cardiovascular disease (CVD) in patients with psoriasis (PS). Therefore, emphasis has lately been laid on the necessity for clinical evaluation of the risk of CVD in these patients. The systemic inflammatory markers C-reactive protein (CRP) and interleukin- (IL-) 6, which have long been used to predict future CVD in the general population, are increased manyfold in patients with PS. Lipid abnormalities characterized by elevated triglycerides, low HDL cholesterol, and higher concentrations of LDL cholesterol and its oxidized form are also prevalent in patients. There is a need for additional laboratory markers for the assessment of cardiovascular status of patients with PS. Due to frequent comorbid overweight and obesity, biologically active compounds produced by adipocytes may have an impact on monitoring the status of the cardiovascular system of patients with PS. For this purpose, two adipokines, adiponectin and leptin, have been most extensively studied. The review focuses on some inflammatory and oxidative stress aspects in patients with PS through the analysis of the impact of prominent adipokines and oxidized low-density lipoprotein (oxLDL) to assess their eligibility for clinical practice as markers of CVD risk in patients with PS.

The role of uric acid in metabolic syndrome in patients with psoriasis

Background and Design:Psoriasis patients have increased risk of obesity, metabolic syndrome and cardiovascular disease. Uric acid is a metabolic marker associated with metabolic syndrome and cardiovascular diseases. Uric acid levels increase in psoriasis as well. The aim of this study was to investigate the role of uric acid in metabolic syndrome in patients with psoriasis. Materials and Methods: Chronic plaque psoriasis patients who presented to the dermatology outpatient clinics in a university-affiliated training and research hospital and age-and gender-matched healthy individuals were included in the study. Waist circumference, height and weight measurements in both groups were recorded, and body mass index was calculated. Serum uric acid, urea, creatinine, C-reactive protein, fasting blood glucose, high-density lipoprotein cholesterol, total cholesterol, triglyceride and insulin levels were determined. Metabolic syndrome and insulin resistance status were evaluated. The findings were compared statistically. Results: Seventy patients with chronic plaque psoriasis (37 females, 33 males) and 60 healthy individuals (31 females, 29 males) were included in the study. The prevalence of metabolic syndrome and uric acid levels were found to be higher in the psoriasis group than in control group (p=0.003 and p=0.008, respectively). Serum uric acid levels and Psoriasis Area and Severity Index scores were higher in psoriasis patients with metabolic syndrome than in those without metabolic syndrome when psoriasis patients were evaluated separately (p=0.041, p=0.024, respectively). A positive correlation was observed between abdominal circumference and serum uric acid levels in psoriasis patients (p=0.003, r=0.350). AbstractAmaç: Enflamatuvar bir hastalık olan psoriaziste son yıllarda obezite, metabolik sendrom ve kardiyovasküler hastalık riskinin arttığı gösterilmiştir. Ürik asit metabolik sendrom ve kardiyovasküler hastalıklarla ilişkili ve aynı zamanda psoriaziste arttığı bilinen metabolik bir belirteçtir. Psoriazis hastalarındaki artmış metabolik sendrom riskinde ürik asitin rol oynayıp oynamadığını araştırmayı amaçladık. Gereç ve Yöntem: Üniversite afiliye eğitim ve araştırma hastanesi dermatoloji polikliniğinde görülmüş olan kronik plak psoriazisli hastalar ile yaş ve cinsiyet uyumlu sağlıklı kontrol grubu çalışmaya alındı. Hasta ve kontrol grubunun bel çevresi, boy ve kilo ölçümü kaydedildi. Vücut kitle indeksleri hesaplandı. Serum ürik asit, üre, kreatinin, C-reaktif protein açlık glukoz, yüksek yoğunluklu lipoprotein kolesterol, total kolesterol, trigliserid ve insülin değerleri ölçümleri kaydedildi. Hasta ve kontrol grubunda metabolik sendrom varlığı ve insülin direnci değerlendirildi. Sonuçlar istatistiksel olarak karşılaştırıldı.
