Measurement of accommodation after implantation of an accommodating posterior chamber intraocular lens

Abstract: Accommodation after implantation of an accommodating PC IOL should be assessed with several techniques, including subjective and objective, to differentiate true pseudophakic accommodation from pseudoaccommodation. Researchers should be aware of the different variability and consistency of measurements with each technique over time.

“…So it depends on patient cooperation and understanding. Subjective methods were found to be more reliable for measuring apparent accommodation according to Langenbucher et al 16 Many previous researches confirmed that the amplitude of apparent accommodation is correlated with different factors including; depth of focus 17,18 , pupil diameter 4,19 , degree of corneal astigmatism 20 , corneal multifocality and aberrations 21,22 , and IOL axial forward movement. 23,24 Depth of focus is an intrinsic characteristic of all optical systems that was considered to be one of the most significant factors of apparent accommodation.…”

“…Accommodation can also be measured subjectively and objectively in pseudophakic eyes. [53][54][55] In pseudophakic eyes, indirect measurements using ultrasonic A-scan measurements 56 and PCI 53,57 before and after ciliary muscle contraction using pilocarpine or phenylephrine found changes in the ACD, suggesting shifting of the IOL. The rate of the shift is highly dependent on the measuring technique and, with these indirect measurements, it is rather the maximum accommodation potential that is being measured.…”

“…The ACD can be measured with different ophthalmological techniques, such as ultrasound biometry, high-resolution magnetic resonance imaging, ultrasound biomicroscopy, Scheimpflug imaging, anterior segment optical coherence tomography, and partial coherence interferometry. 31,33,34 We used objective measurements with a newly developed high accuracy interferometric device.…”
