Maximal Exercise Testing of Men with Prostate Cancer Being Treated with Androgen Deprivation Therapy

Abstract: Maximal exercise testing was demonstrated to be feasible and safe, providing a direct assessment of V˙O2max. The relatively low number of positive tests in this study suggests that the risk of adverse events is relatively low in this population and certainly no higher than that in age-matched, apparently healthy individuals.

“…To examine cardiovascular adaptations to exercise, oxygen consumption (VO 2 ) and heart rate (HR) responses were measured during the first 2 stages of a modified Bruce Treadmill protocol. 13 These stages correspond to velocities of 2.7 and 4.0 km/h at gradients of 10% and 12%, respectively. Oxygen uptake was measured through indirect calorimetry (Parvo Metabolic Measuring System, Sandy, UT).…”

“…In the interim, in men undergoing ADT, the monitoring and management of cardiovascular risk factors (hyperglycemia, hyperlipidemia, hypertension, and obesity) should be seen as an integral part of patient management. 74 …”

“… 75 A further prospective randomized trial of dietary and exercise intervention is underway, 76 but while these important studies may yield benefits to prostate cancer patients, they are unlikely to be able to demonstrate whether such interventions are capable of ameliorating the risks associated with ADT. 74 77 What is clear is that in the general population, cardiac rehabilitation programs that primarily utilize exercise produce compelling and consistent clinical results in men with preexisting cardiovascular disease. Randomized trials have shown that cardiac rehabilitation reduces the probability of suffering additional cardiac events and is associated with a broad range of benefits, including reduced mortality.…”

“…A total of 60 patients had been <3 months with ADT (acute), while 52 had been on ADT for >3 months (chronic). Average VO 2 max values were 24.7 ± 6.0 mL/kg/min . Caffeine appears to improve exercise tolerance through improved performance without subsequently increasing fatigue or the perception of effort.…”

“…Thus, interventions to improve exercise motivation and adherence are of great concern. Wall et al evaluated 112 patients with prostate cancer undergoing ADT with a maximal stress test (Bruce protocol). A total of 60 patients had been <3 months with ADT (acute), while 52 had been on ADT for >3 months (chronic).…”
