Objective: To describe obstetric practices in planned home births, assisted by qualified professionals in Brazil. Method: This is a descriptive study, with data collected in an online bank maintained by 49 professionals from December 2014 to November 2015, in which the target population was women and newborns assisted in home births. Data were analyzed through descriptive statistics. Results: A total of 667 women and 665 newborns were included. Most of the women gave birth at home (84.4%), in a nonlithotomic position (99.1%); none underwent episiotomy; 32.3% had intact perineum; and 37.8% had first-degree lacerations, some underwent amniotomy (5.4%), oxytocin administration (0.4%), and Kristeller’s maneuver (0.2%); 80.8% of the women with a previous cesarean section had home birth. The rate of transfer of parturients was 15.6%, of puerperal women was 1.9%, and of neonates 1.6%. The rate of cesarean section in the parturients that started labor at home was 9.0%. Conclusion: The obstetric practices taken are consistent with the scientific evidence; however, unnecessary interventions are still performed. The rates of cesarean sections and maternal and neonatal transfers are low. Home can be a place of birth option for women seeking a physiological delivery.

“…Um estudo realizado em Santa Catarina em 2017, com uma população de 187 parturientes de risco habitual, que demonstrou os resultados maternos de partos domiciliares, apresentou uma taxa de episiotomia inferior ao que foi encontrada no presente estudo. Houve apenas uma episiotomia, representando 0,5% do total de partos assistidos, 27,3% períneo íntegro e 72,2% de lacerações perineais, sendo que destas 72,1% foram de primeiro grau, 7,9% de segundo grau e não houve lacerações de terceiro e quarto grau (15) .…”

Prática De Episiotomia Entre Residentes Em Enfermagem Obstétrica

Objetivo: identificar a frequência e justificativa para a realização da episiotomia em partos assistidos por residentes em enfermagem obstétrica. Método: estudo descritivo e retrospectivo, realizado no período de outubro a novembro de 2016, em uma maternidade pública do município de São Paulo. A população do estudo foi constituída por 884 parturientes de baixo risco. Para análise estatística, utilizou-se o teste Qui-Quadrado. Resultados: a episiotomia ocorreu em 174 (19,7%) partos e em 512 (59%) houve lacerações perineais. A integridade perineal foi mantida em 187 (21,4%) partos. As principais indicações estiveram relacionadas às condições do períneo: 54 (58,1%) por rigidez perineal, 22 (23,7%) períneo curto e 19 (20,4%) eminência de laceração grave. Conclusão: a prática de episiotomia entre residentes está acima do recomendado pela OMS e, com relação às justificativas apresentadas, há discrepância com as mundialmente utilizadas, trazendo reflexões acerca do modelo de formação e do distanciamento entre teoria e prática.

Background The American College of Obstetricians and Gynecologists, in its opinion of the Committee on Midwifery Practice, points out that planned home birth is a woman's and family's right to experience, but also to choose and be informed about, their baby's place of birth. The aim of this study was to understand obstetric nurses' perceptions of planned home childbirth care within the framework of the Brazilian obstetric model. Method A qualitative study, with Snowball Sampling recruitment, totaling 20 obstetric nurses through semi-structured interviews between September 2022 and January 2023, remotely, using the Google Meet application and the recording feature. After the data had been collected, the material was transcribed in full and subjected to content analysis in the thematic modality with the support of ATLAS.ti 8.0 software. Results Obstetric care at home emerged as a counterpoint to hospital care and the biomedical model, providing care at home based on scientific evidence and humanization, bringing qualified information as a facilitator of access and financial costs as an obstacle to effective home birth. Conclusion Understanding obstetric nurses' perceptions of planned home birth care in the context of the Brazilian obstetric model shows the need for progress as a public policy and for strategies to ensure quality and regulation.
