Background: Placenta is the vital link between fetus and the uterine wall. Based on location, it can be classified as anterior, posterior, lateral, fundal and low lying. Placental location can be easily determined in the antenatal period using ultrasound. Materials and Methods: Design: Prospective observational study. Study period: One year from January 2018 to February 2019 in a tertiary hospital in Salem district of Tamilnadu. Study population: After informed consent, 200 women with singleton pregnancy of ≥28 weeks attending antenatal OPD were included in the study. Women were followed up with ultrasound at 28 weeks and 34 weeks to identify any maternal and fetal adverse effects such as gestational hypertension, preeclampsia, preterm birth and NICU admission. Results: The mean age of the study participants was 24.3 ± 43 years. Of the 200 women, 42% (n=84) of placenta were situated in fundus, 30% (n=60) were anterior, 18% (n=36) were lateral, 8% (n=16) were posterior and 2% (n=4) were low lying. Gestational hypertension was present in 22%, 13%, 12% in lateral, anterior and posterior placental location respectively. Preeclampsia was seen in 22% in lateral placental location. Preterm labour, PROM, low birth weight and NICU admission were common in posterior and lateral placental location.Conclusion:There was statistically significant association between low lying, posterior and lateral placental location and adverse maternal fetal outcomes. Hence ultrasound can be used as the safe, cost effective, non-invasive tool in predicting the adverse maternal and neonatal outcome.

Introduction: Placenta previa is one of the major causes of antepartum hemorrhage and a life-threatening condition for both the mother and the fetus. The incidence of placenta previa is 0.3 to 1% in 300 to 400 deliveries. [1] The present study was done retrospectively to observe the maternal and fetal outcome in pregnancies with placenta previa. Method:The retrospective study was done from June 2019 to June 2021 to study the maternal and neonatal outcome in pregnancies with placenta previa who got admitted in Obstetrics and Gynecology department of Rajindra Hospital, Government Medical College Patiala. Detailed history, clinical examination and investigations were evaluated and analysis was done at the end of the study.Result: Total number of deliveries during this period were 8193 out of which placenta previa was present in 210 with an incidence of 2.5%.The major risk factor for placenta previa in the present study was previous LSCS in 34.3% followed by previous dilatation and curettage (D&C) in 16.6%. The majority of the women 57.2% had major degree placenta previa, out of which placenta accreta was present in 5.8%. The majority of the women 94.7% were delivered by LSCS and 4.2% had obstetric hysterectomy, 16% had PPH intraoperatively and 1.4% needed ICU care. 60% of the babies were born prematurely and 25.7% required NICU care. Conclusion:Placenta previa is a major contributor in increasing maternal and fetal morbidity and mortality. The present study showed that increasing maternal age, multiparity, previous cesarean section, previous dilatation and curettage are the risk factors for placenta previa. Placenta previa is associated with increased intraoperative interventions, surgical complications and maternal morbidity.

Objective: The aim of the study was to evaluate maternal and fetal outcomes among patients of placenta previa (PP) with and without placenta accreta (PA). Methodology: All patients who underwent cesarean section for PP and PA were analyzed retrospectively at a tertiary care Combined Military Hospital Kharian, Pakistan, from February 2015 to March 2018. Maternal and neonatal data were obtained from medical records and the hospital database system. Results: PA was found in 37 patients from 111 patients of PP and 74 were without PA with the rate of approximately 2/1000 and 4/1000 respectively were included in the study. The mean age was 31.16±2.65 (range 22–37) years, mean gravidity of 3.69 ±1.40 (range 1 - 9), mean parity 2.57±1.01 (range 1–5), mean number of cesarean sections 2.10±0.66, (range 1-3) and a mean gestational age at the time of cesarean section was 35.65±2.46 (range 28–41) weeks. The maternal risk factors revealed marked differences between placenta previa with accreta and without accrete. The mean intraoperative blood loss in PA was 3,000ml, with a loss of 2,000ml occurring in 60%, and 3,000 ml in 21% of the PA cases. The mean pRBC transfusion was 4 units, while 17% received 6 units. Fetal growth restriction was not seen. A total of 12 neonates were admitted in NICU, with 03 neonatal deaths. There was no maternal death. Neonates born to women with placenta accreta had significantly lower birth weight, Apgar scores at 1 min and 12% required admission to NICU with 3 neonatal deaths. Conclusion: The advanced maternal age, past cesarean or uterine surgery, high parity as well as multiple gravidity were the risk factors for adverse fetal and maternal outcomes.
