Purpose of the StudyPatients undergoing ablative tumor surgery of the midface are faced with functional and esthetic issues. Various reconstructive strategies, such as implant-borne obturator prostheses or microvascular tissue transfer, are currently available for dental rehabilitation. The present study shows the first follow-up of patients treated with patient-specific implants (IPS Implants® Preprosthetic) for the rehabilitation of extended maxillary defects following ablative surgery.Patients and MethodsAll patients treated with patient specific implants due to postablative maxillary defects were included. 20 implants were placed in the 19 patients (bilateral implants were placed in one of the cases). In 65.75% of the cases, resection was performed due to squamous cell carcinoma. In addition to the primary stability, the clinical implant stability, soft tissue management, successful prosthodontic restoration, and complications were evaluated at a mean follow-up period of 26 months.ResultsAll patient-specific implants showed primary stability and were clinically stable throughout the observation period. Definitive prosthodontic restorations were performed in all patients. No implant loosening was observed. Major complications occurred only in previously irradiated patients with insufficient soft tissue conditions (p = 0.058). Minor complications such as exposure of the underlying framework or mucositis were observed, but they never led to failure of restorations or implant loss.ConclusionsTreatment of postablative maxillary defects with patient-specific implants offers a safe alternative with predictable results for full and rapid dental rehabilitation, avoiding time-consuming augmentation procedures and additional donor-site morbidity.

Tümör rezeksiyonu sonucu geniş maksiller defekti bulunan 45 yaşındaki erkek ve 57 yaşındaki kadın hastanın bulb obturatörler ile gerçekleştirilen protetik rehabilitasyonlarının sunulması amaçlanmıştır. Olgu Sunumu: Maksillalarında skuamöz hücreli karsinom bulunan erkek hasta ile mukoepidermoid karsinom bulunan kadın hastanın, parsiyel maksillektomi operasyonları sonrası protetik olarak rehabilite edilmeleri planlanmıştır. Bu doğrultuda, defekt bölgelerinden alınan ölçüler ile erkek hastaya bulb obturatör, kadın hastaya hollow bulb obturatör uygulanmış; her iki hastanın parsiyel dişsizliği bulunan alt çeneleri ise metal iskeletli parsiyel protezler ile rehabilite edilmiştir. Oronazal açıklığı bulunan erkek hastanın rezonans ve beslenme probleminin obturatör protez ile giderildiği, üst dudağın desteklenmesiyle birlikte yüz estetiğinin iyileştirildiği gözlenmiştir. Kadın hastaya uygulanan hollow bulb obturatör ile de hasta konforunun artırıldığı tespit edilmiştir. Sonuç: Erkek hastanın 6, kadın hastanın ise 12 aylık takibinde; rezeksiyon sonrası ortaya çıkan fonksiyon, fonetik ve estetiğe dair şikayetlerin giderildiği gözlenmiştir. Bu tip büyük defektlerde implant üstü protezlerin daha yüksek düzeyde retansiyon ve stabilizasyon sağladığı bilinmektedir. Ancak genel sağlık problemleri, maddi durumlar, kontrendikasyon vb. sebeplerle implant tedavilerinin uygulanamadığı durumlarda; konvansiyonel obturatör protez rehabilitasyonları güvenle uygulanabilmektedir.

Oral Squamous Cell Carcinoma of the Maxilla

Purpose: For planning of the surgical resection, computed tomography (CT) and magnetic resonance imaging (MRI) are commonly used for the preoperative assessment of bone invasion of the maxilla.The purpose of this study was to compare the diagnostic test accuracy of CT and MRI for detecting bone invasion of the maxilla in patients with squamous cell carcinoma of the maxilla (MSCC). Materials and Methods:We conducted a retrospective cross-sectional study and enrolled a consecutive number of patients with primary MSCC between 2000 and 2017 who underwent either preoperative CT or MRI scans. The outcome variable was the absence or presence of bone invasion, with histopathologic examination of the resection specimen as the gold standard. The predictor variable was the imaging technique (CT and MRI). The imaging results on bone invasion were compared with the histopathologic results. Sensitivity and specificity were calculated, and the 2-sided Fisher exact test was used to calculate statistically significant differences between the unpaired CT and MRI results. Receiver operating characteristic curves were computed, and the area under the curve (AUC) was calculated. Results:The study included 72 patients (29 male and 43 female patients) with a mean age of 72 years.A total of 41 CT scans and 31 MRI scans were available. Histopathologic examination showed bone invasion in 45 cases: 26 of 41 patients with CT scans (63%) and 19 of 31 patients with MRI scans (61%).CT yielded 2 false-positive and 2 false-negative results, with a sensitivity of 92%, specificity of 87%, and AUC of 0.895. MRI yielded 5 false-positive and 2 false-negative results, with a sensitivity of 89%, specificity of 58%, and AUC of 0.739. No significant differences were observed for sensitivity (P > .999) and specificity (P = .185). Conclusions:In the absence of metallic dental restorations, CT could detect bone invasion more accurately than MRI in this study; however, the difference was not statistically significant. The imaging method of choice may depend on other situational factors.
