Background Combination of dyslipidemic phenotypes, including elevated plasma levels of low‐density lipoprotein cholesterol (LDL‐C), elevated plasma triglycerides (TG), and decreased low‐density lipoprotein cholesterol (HDL‐C) concentrations, is important because of the association of individual phenotypes with increased risk of cardiovascular disease (CVD). We investigated the prevalence of combined dyslipidemias and their effects on CVD risk in an Iranian large population. Method A total of 9847 individuals were recruited as part of the Mashhad Stroke and Heart Atherosclerotic Disorders (MASHAD) cohort study. Anthropometric parameters and biochemical indices were measured in all of the subjects. Different types of combined dyslipidemias including high TG + low HDL‐C, high TG + low HDL‐C + high LDL‐C, low HDL‐C + high LDL‐C, high TG + high LDL‐C, and finally high TG + high LDL‐C + low HDL‐C were considered. Ten‐year CVD risk was calculated using the QRISK2 risk algorithm and adjustments were made as suggested by the Joint British Societies’ (JBS2). Logistic regression analyses were performed to determine the association between different combined dyslipidemias and categorical QRISK. Results A total of 3952 males and 5895 females were included in this current study. Among the included subjects, 83.4% had one form of dyslipidemia, and 16.6% subjects were not dyslipidemic. The mean age was 48.88 ± 7.9 and 47.02 ± 8.54 years for dyslipidemic and nondyslipidemic groups, respectively. The results showed that the frequency of dyslipidemia was 98%, 87.1%, and 90% in subjects with metabolic syndrome, CVD, and diabetes, respectively. Our results suggested that around 15.7% of study population were at 10 years CVD risk (high ≥20) and it was higher in men than women (P < .001). Moreover, risk of CVD was higher in TG↑ & HDL↓ & LDL↑ group than other groups. Conclusion Prevalence of dyslipidemia was 83.4% among Iranian adults. The results showed that individuals with increased plasma TG and LDL‐C, and low HDL‐C levels had the highest 10 years CVD risk compared to other combined dyslipidemic phenotypes.

Human T lymphotropic virus type 1 and risk of cardiovascular disease: High‐density lipoprotein dysfunction versus serum HDL‐C concentrations

High-density lipoprotein (HDL) is thought to be protective against cardiovascular disease (CVD), and HDL dysfunction is considered to be a risk factor for CVD. It is unclear whether there is an association between Human T lymphotropic virus type 1 (HTLV1) infection and CVD risk. We have assessed HDL lipid peroxidation (HDLox) as a marker of HDL dysfunction and CVD risk in a subgroup of the MASHAD cohort study. One hundred and sixty two individuals including 50 subjects positive for HTLV1 infection and 112 individuals negative for HTLV1 infection were recruited. Anthropometric and biochemical parameters including serum hs-CRP, fasted lipid profile (HDL-C, LDL, triglycerides, and cholesterol), and fasting blood glucose were determined. Serum HDLox was also measured in the study participants. Multivariate analyses were used to evaluate the association between serum HDLox and HTLV1 infection. None of the traditional CVD risk factors were associated with HTLV1 infection, including serum HDL-C. However, serum HDLox was independently associated with the presence of HTLV1 infection. Logistic regression analysis showed that subjects who were positive for HTLV1 infection were also significantly more likely than uninfected individuals to have higher HDLox (odds ratio 9.35, 95%CI: 3.5–24.7; P < 0.001). HDLox was increased approximately 20% (P < 0.001) in infected subjects compared to the uninfected group. Serum HDLox is a marker of CVD risk factor and increased in individuals affected by HTLV1 infection compared to healthy subjects.

BACKGROUND AND AIM: Obesity is a major risk factor for type 2 diabetes mellitus and cardiovascular disease (CVD). Little is known about the metabolic profile across categories of body mass index (BMI), particularly within Asian populations. We assessed the prevalence and distribution of body size phenotypes among Iranian adults from a representative sample from Mashhad. METHOD: The Mashhad Stroke Heart Atherosclerosis Disorder (MASHAD) study cohort of 9761 adults (3903 men) aged 35-65 years were cross-categorized by BMI and metabolic status into: normal-weight metabolically healthy (NWMH), normal-weight metabolically abnormal (NWMA), overweight metabolically healthy (OvMH), overweight metabolically abnormal (OvMA), obese metabolically healthy (OMH) and obese metabolically abnormal (OMA). Metabolic normality/abnormality was defined by the presence of any two of the following: 1) elevated blood pressure or known hypertension on treatment, 2) high serum fasted triglycerides; 3) low serum HDL-cholesterol, 4) high blood glucose or the presence of diabetes mellitus, 5) serum CRP > 90th percentile. RESULTS: The distribution of obesity phenotypes was 11.6% (NWMH), 15.6% (NWMA), 10.6% (OvMH), 31.3% (OvMA), 4.9% (OMH), and 25.5% (OMA) overall. Equivalent figures were 13.1%, 24.0%, 7.7% 37.3%, 1.5% and 16.2% in men; 10.7%, 10.2%, 12.5%, 27.4%, 7.2% and 31.8% in women, p < 0.001 for gender differences.
