Odontogenic cysts of the jaws are one of the most common tumour-like neoplasms of the maxillofacial region in children. Today, there are various methods of treating cysts, but in childhood, the problem of choosing a treatment method is relevant and at the same time difficult, as some authors prefer cystectomy, while others prefer cystotomy. Purpose - to analyze the methods of treatment and management of jaw cysts in children depending on the type of cyst. Materials and methods. A retrospective analysis of 286 case histories of patients with jaw cysts aged 4 to 17 years was performed. The case histories were analysed according to the developed examination chart. Results. A retrospective analysis of 286 case histories of children with jaw cysts was performed. The age of the children ranged from 4 to 18 years. Radicular dentoalveolar cysts were diagnosed in 35% (n=100) of cases, radicular cysts - in 36% (n=104), follicular cysts - in 15% (n=43), suppurative cysts - in 5% (n=14), residual cysts - in 5% (n=14), primary bone cysts - in 3% (n=7), polycystic cysts - in 2% (n=4). Cystotomy was performed in the vast majority of cases - 58% (n=166), cystectomy - in 42% (n=120). Tamponade was performed in 63% (n=181) of cases, and in 37% (n=105) it was not performed. Conclusions. Radicular cysts are the most common odontogenic cysts of the jaws. Cystotomy and cystectomy are variable methods of cyst treatment. The most common method of treating cysts in children is cystotomy. The research was carried out in accordance with the principles of the Helsinki Declaration. The study protocol was approved by the Local Ethics Committee of the participating institution. The informed consent of the patient was obtained for conducting the studies. No conflict of interests was declared by the authors.

Jaw cysts represent a great matter of interest in oral and maxillofacial surgery. Jaw cyst surgery is a common intervention in oral surgery but, in the case of a multidisciplinary patient, the oral surgeon needs to meet with other specialists. A cyst is an epithelium-lined sac containing fluid and/or semisolid material due to epithelial cell proliferation, degeneration, and liquefaction; the hypertonic solution withdraws liquids from the surrounding tissues, while internal pressure exerts an equal strength on the cyst walls. Dentigerous cysts are the second most common odontogenic cysts after radicular cysts, and commonly few or no symptoms are reported. However, the most common diagnosis for dentigerous cyst is represented by eruption of the affected tooth or accidental diagnosis. Commonly, dentigerous cysts may be related to impacted third molars; in the case of impacted third molars and a dentigerous cyst, the tooth should be removed along with the cyst in the same intervention. Mandibular dentigerous cysts are common in children and adults, while dentigerous cysts are a rare neoformation in elderly patients. Treatment usually involves removal of the entire cyst and the associated unerupted tooth. This intervention may be more difficult if the cyst is large, the third molar is in contact with the mandibular nerve, and/or the patient has a medical history that may represent a relative or absolute contraindication. We present the case of a rare symptomatic manifestation of dentigerous cyst in an elderly patient in treatment with dabigatran therapy; for the treatment of dentigerous cysts in the elderly, we suggest a multidisciplinary approach with the use of the histological examination and a careful follow-up.

This study aimed to review and critically appraise the available literature concerning marsupialization's effectiveness in reducing cystic jaw lesions in children. The study conducted a search strategy that involved combining the following keywords: (1) children, (2) marsupialization, (3) jaw, and (4) cyst. An electronic search on four critical databases—PubMed, The Cochrane Library, Science Direct, and ResearchGate—to review English‐language articles with no limitations in terms of time, sample size, or type of study design has been conducted. Based on the inclusion and exclusion criteria applied to the preliminary search, 46 studies were included in the final review. Among the 194 identified lesions, the majority were dentigerous cysts (93.8%), followed by radicular cysts (2.6%), odontogenic keratocysts (1.6%), juvenile paradental cysts (1%), and calcifying odontogenic cysts (1%). Postoperatively, permanent teeth involved in the cysts were reported to develop continuously in 40 studies, whereas bone neo‐formation after removal of cystic lesions was reported in 30 studies. Only one study reported postoperative recurrence. The complete healing time of the cysts after marsupialization ranged from 6 months to 5 years. Marsupialization was mainly considered a definite treatment. Because of the long follow‐up period, the cooperation of the patient and family plays an essential role in the success of treatment.
