PurposeThe aim of this study was to compare the location and the shape of the mandibular lingula in skeletal class I and III patients using panoramic radiography and cone-beam computed tomography.Materials and MethodsThe sample group included 190 skeletal class I patients and 157 class III patients. The location of the lingula in relation to the deepest point of the coronoid notch was classified into 3 types using panoramic radiographs. The shapes of the lingulae were classified into nodular, triangular, truncated, or assimilated types using cone-beam computed tomographic images. The data were analyzed using the chi-square test.ResultsThe tips of the lingulae were at the same level as the coronoid notch in 75.3% of skeletal class I patients and above the coronoid notch in 66.6% of class III patients. The positions of the lingulae in relation to the deepest point of the coronoid notch showed statistically significant differences between class I and class III patients. The most common shape was nodular, and the least common was the assimilated shape. Although this trend was not statistically significant, the triangular shape was more frequently observed in class III patients than in class I patients.ConclusionThe locations and the shapes of the mandibular lingulae were variable. Most of the lingulae were at the same level as the coronoid notch in skeletal class I patients and above the coronoid notch in skeletal class III patients. The nodular and assimilated-shaped lingulae were the most and the least prevalent, respectively.

INTRODUÇÃO: A mandíbula é um dos ossos da face mais susceptíveis à fraturas e deformidades, tratados através de procedimentos cirúrgicos. Na técnica de osteotomia sagital do ramo mandibular (OSRM) o corte horizontal é feito na face medial do ramo imediatamente acima da língula. OBJETIVO: O propósito desse estudo foi verifi car a existência de correlação da altura e largura do ramo em relação ao gênero e índice cefálico. MATERIAL E MÉTODO: Foram utilizadas 48 mandíbulas humanas dentadas de adultos do tipo dolico (16), meso (16) e braquicefálico (16). Os pontos pré determinados utilizados como referências anatômicas no ramo para mensurações foram: (I) ponto mais inferior da incisura mandibular, (B) ponto mais alto da margem inferior, (A) ponto mais posterior da margem anterior (A) e (P) ponto mais anterior da margem posterior. RESULTADOS: A média da altura e largura do ramo mandibular não apresentou diferença na comparação entre os índices cefálicos (dolico - 47,05 / 32,09mm; meso - 47,27 / 32,57mm; braquicefálico - 46,41 / 29,58mm). Na comparação das dimensões com o gênero, a diferença encontrada foi a altura do ramo no sexo masculino (49,08mm), maior do que no feminino (42,76mm). CONCLUSÕES: Os resultados aqui encontrados podem ser utilizados como referência em procedimentos cirúrgicos do ramo mandibular, investigações forenses e estudos de comparação a dados obtidos em espécimes de outras nacionalidades.

Horizontal osteotomy is one of the most critical step at sagittal split ramus osteotomy (SSRO) and determination of the ideal height of this horizontal osteotomy is essential to avoid nerve and vessel injury.Purpose: The aim of this study was to evaluate the level of the medial horizontal ramus cut as a risk factor for unfavorable outcomes in the SSRO. Materials and Methods: Sixty-four patients with dentofacial deformity who applied to Oral & Maxillofacial Surgery Department between August 2018 and August 2019 and undergone orthognathic surgery were evaluated. Out of 64, 49 patients had SSRO with or without maxillary surgery and genioplasty. Twenty-six patient had postoperative computed tomography scan with 6-months follow-up. Finally, 26 patient with 52 SSRO sides were included in this study. Computed tomography scans were evaluated and classification according to osteotomy levels was made. Postoperative neurosensory deficit, bleeding, and intraoperative complications such as bad split, visible damage to inferior alveolar bundle were assessed. Age, gender, neurosensory deficit, bad splits were analyzed and correlated with the level of the osteotomies. Results: Fifteen osteotomies were above lingula, 24 between apex and base of lingula, and 14 below lingula. One bad split occurred, and no visible damage to the inferior alveolar bundle was seen. There was no significant difference between osteotomy groups in terms of visual analogue scale (VAS) scores (P > 0.05) but in all groups; women's VAS scores are statistically significantly higher than men. (P: 0.036) Conclusion: There is no correlation between the horizontal osteotomy level and intraoperative or postoperative complications. The low medial horizontal osteotomy can be safely performed in SSRO.
