ÖZETAmaç: Bisfosfonat tedavisi altında olan bireylerin mandibular kemik dokusu değişikliklerini fraktal analiz yöntemi ile saptayıp ve sağlıklı bireylerden elde edilen bulgular ile karşılaştırmaktır. Yöntem: 20 adet sağlıklı 20 adet bisfosfanat tedavisi altında olan hastadan elde edilen görüntüler üzerinde kortikal kalınlık (KK), panoramik mandibular indeks (PMI) ve fraktal boyut (FB) ölçümleri yapıldı. FB ölçümleri, her hastaya ait panoramik görüntü üzerinde 3 farklı alanda (angulus, corpus ve interdental kemik) gerçekleştirildi. İstatistiksel analiz aşamasında ANOVA, Tukey-Kramer testi ve Pearson korelasyon katsayısından yararlanıldı. Bulgular: 2 farklı hasta grubuna ve farklı lokalizasyonlara ait FB değerleri arasında istatistiksel olarak anlamlı bir farklılık saptandı (p=0.0004). İnterdental kemik ve korpus mandibula bölgesin ait FB değerleri arasındaki fark anlamlı iken (p=0.008), diğer 2 farklı lokalizasyona ait değerlerin ikili karşılaştırmalarında anlamlı bir farklılık saptanmadı (p>0.05). 2 farklı hasta grubunun KK ve PMI değerleri arasında anlamlı bir farklılık bulunmadı (p>0.05). KK-PMI( p=0.86) ve KK-FB ( p=0.26) arasında düşük bir korelasyon saptanırken, FB ve PMI arasında negatif bir korelasyon saptandı (p=0.96). Sonuç: Fraktal boyut analizi, bisfosfanat tedavisi altındaki hastaların mandibular kemik dokusu değişikliklerinin ayırımında kullanılabilecek bir yöntemdir. Direkt dijital panoramik sistem kullanılarak elde edilen görüntüler üzerinde interdental kemik ve korpus mandibula bölgesine ait gerçekleştirilen FB ölçümlerinden bisfosfanat kullanan hastaların izlenmeleri aşamasında yararlanılabilir. Résumé -Introduction : L'essor de l'implantologie au cours des dernières années a conduit à une augmentation du champ de ses indications, repoussant notamment les contre-indications liées aux terrains pathologiques des patients. Cet article propose une revue de la littérature et une mise au point sur l'implantologie dans un contexte de pathologies de la muqueuse buccale. Méthodologie : À partir d'une recherche bibliographique sur la base de données MEDLINE sans limite de période, les articles traitant de l'implantologie chez des patients atteints de pathologies de la muqueuse buccale ont été étudiés. Résultats : L'implantologie chez ces patients a été peu décrite. Les données de cette revue de la littérature tendent à mettre en évidence un apport indéniable de la mise en place d'implants chez les patients atteints de pathologies de la muqueuse buccale. Discussion : Historiquement considérées comme des contre-indications à la pose d'implants, ces pathologies ne doivent plus aujourd'hui être considérées comme un frein à une réhabilitation prothétique implanto-portée. Conclusion : Toutefois, une maintenance et une hygiène rigoureuses doit être instaurée afin de contrôler les implants et l'éventuelle réactivation de la pathologie.Abstract -Implantology and oral mucosal lesions: a literature review. Introduction: Recent developments in implantology have led to an extension of its indications, especially regarding special care patients. This paper offers a review of the literature on the subject of implantology in cases of oral mucosal lesions. Methods: A review of the literature was performed using MEDLINE, without a date range, searching for articles describing oral implant treatments in patients with oral mucosal alterations. Results: Few cases of oral implant treatments in such patients are described. The results of the literature review showed that oral implant treatments greatly enhance the quality of life of these patients. Discussion: Although these pathologies used to be considered contraindications to dental implant placement, recent data shows the feasibility of such treatments. Conclusion: Nonetheless, both strict oral hygiene and regular maintenance are essential to intercept any potential pathology reactivation.
