Peroral endoscopic EHL or ILL, under direct cholangioscopic visualization by a mother-baby endoscopic system, is an effective treatment for difficult CBD stones. The technique can be used safely even in frail and elderly patients. However, several endoscopic attempts may be required before final stone clearance is achieved. The vast majority of patients may be expected to remain symptom free for a prolonged period.

El 85% a 90% de los cálculos de la vía biliar son manejados de forma exitosa con la técnica endoscópica es- tándar, que incluye esfinterotomía endoscópica combinada con canastilla y/o catéter con balón. Sin embargo, cuando los cálculos son múltiples, de gran tamaño, de localización o forma inusual, o existen alteraciones anatómicas de la vía biliar se hacen refractarios al manejo estándar, lo que obliga la realización de otras modalidades terapéuticas. Los cálculos de gran tamaño o impactados generalmente son manejados con técnicas de fragmentación como la litotripsia mecánica y en casos fallidos con litotripsia electrohidráulica (LEH) o litotripsia láser (LL) guiado por colangioscopia convencional o más recientemente por un sistema de colangioscopia directa llamado Spyglass®. También se puede usar el sistema de dilatación endoscópica papilar con balón grande, que ha demostrado ser útil en el manejo de cálculos grandes y múltiples. En casos con anatomía alterada con dificultad para acceder a la papila, el abordaje preferido para la técnica de frag- mentación es el percutáneo transhepático. En pacientes ancianos y con pobre estado general la colocación de una prótesis biliar es la técnica de elección definitiva mientras mejoran las condiciones del paciente para un tratamiento endoscópico posterior.

Endoscopic Treatment of Choledocholithiasis Nowadays, treatment of common bile duct stones is usually performed by endoscopic sphincterotomy and stone removal which is an effective, efficient and safe procedure with a very low mortality. Therefore, endoscopic stone removal is the treatment of choice for patients with acute cholangitis and biliary pancreatitis. After achieving an access to the common bile duct by sphincterotomy or balloon dilation of the sphincter Oddi, endoscopic duct clearance by Dormia basket or balloon catheter is possible in at least 90%. Additional techniques including mechanical lithotripsy, extracorporeal shock wave lithotripsy, electrohydraulic and laser lithotripsy allow removal of almost all ‘difficult’ bile duct stones that are refractory to standard extraction due to their size or impaction. Endoscopic treatment of difficult biliary stones should combine several therapeutic options for maximum duct clearance and short procedure time, particularly in elderly patients. Despite the use of all endoscopic techniques, stone removal fails in 1–2%. In those rare cases, patients still require elective surgery or palliative endoprosthesis implantation. Especially younger patients with additional cholecystolithiasis should undergo cholecystectomy.
