Background: The goal of treatment in displaced lateral condylar fracture is to achieve union without residual deformity. However, growth disturbance may occur despite initial anatomic reduction and secure fixation. Objectives: The purpose of this study was to evaluate clinical and radiographic results and assess complications associated with the operative treatment of lateral condylar fractures in children. Methods: A total of 73 pediatric patients under the age of 13 years, who were diagnosed with displaced lateral condylar fractures of the humerus and were treated with open reduction and internal fixation, were assessed from March 2011 to September 2015. The mean follow-up duration in these patients was 22 months (8 -48 months). The clinical results were evaluated using the Hardcare scoring system. Also, anteroposterior and lateral radiographic views were assessed to identify the union process and presence of complications. Results:The results of evaluation on 48 boys and 25 girls demonstrated that lateral condylar fractures of the humerus were more common in boys, aged 5-8 years (average: 57.1%). There was no significant relationship between complications and sex. However, according to the radiographic evaluation, while fractures in 71 (97.2%) patients were completely united without any complications, 2 (2.8%) patients experienced overgrowth of the lateral humeral condyle. Clinical evaluation demonstrated excellent and good outcomes in 68 and 5 patients, respectively. Also, evaluation of Hardcare scores revealed no significant relationship between complications and clinical outcomes. Conclusions:The present results were in line with previous research regarding the absence of complications such as non-union, avascular necrosis, fishtail deformity, cubitus varus, cubitus valgus, and pain. The findings demonstrated that the acceptable treatment option for displaced and rotated fractures is open reduction and internal fixation. Furthermore, deformity did not affect the clinical outcomes of fractures.

La luxation négligée du coude chez l'enfant est peu fréquente dans les pays industrialisés, elle n'est pas en revanche rare dans notre contexte. Nous rapportons une série de 20 cas, colligée au service d'Orthopédie Pédiatrique du CHU HASSAN II de Fès, sur une période de 4 ans. En effet, 14 malades étaient de sexe masculin, avec un âge moyen de 8,5 ans. Le mécanisme principal était la chute d'olivier. Le délai de consultation était très variable. 19 enfants ont été traités traditionnellement. La raideur constituait le motif principal de consultation. Avec une flexion inférieure à 70°. Un seul malade avait présenté un syndrome de Volkmann. La luxation était isolée et postéro-externe dans la majorité des cas. La voie d'abord médiane postérieure était adoptée chez tous nos malades. 15 patients ont bénéficié d'une arthrolyse avec arthrodèse provisoire. La rééducation était systématique. Le résultat a été bon avec une flexion à 90° et un secteur de mobilité supérieur à 60° chez 9 de nos malades sur un recul de 3 ans. En définitive, la luxation invétérée du coude est une pathologie du milieu rural; la facilité d'accès au soin reste la meilleure prévention.

Objective To evaluate the clinical efficacy of two surgical methods in the treatment of fracture of lateral condyle of humerus in children and the application value of two kinds of internal fixation materials.Methods A total of 32 children with fracture of lateral condyle of humerus were treated in our hospital from June 2015 to June 2018, with an average age of 5.6 years, including 18 males and 14 females. The patients were divided into two groups: group A (n = 12) and group B (n = 20). By comparing the operation time, removal of internal fixation time, Dhillon elbow function evaluation, HHS elbow function score, X-ray measurement of postoperative carrying angle, Baumann angle and distal humerus anteversion angle, the operation-related complications were recorded in detail.Results The operation time was 0.9 ±0.2 hours in group A, 0.8 ±0.2 hours in group B, P > 0.05. The removal internal fixation time was 91 ±8 days in group A, 59 ±10 days in group B (P < 0.05). The Dhillon elbow function score was 8.2 ±0.6 in group A, 8.0 ±0.8 in group B, and 93 ±5 in group A and 89 ±4 in group B (P > 0.05). The carrying angle was 10 °±1 °in group A and 10 °±2 °in group B (P > 0.05). The Baumann angle was 15 °±4 °in group A and 16 °±3 °in group B (P > 0.05). The anteversion of distal humerus was 33 °±3 °in group A and 33 °±4 °in group B (P > 0.05). In the last follow-up, the excellent and good rate of elbow joint function in group A was excellent in 8 cases and good in 4 cases, and the excellent and good rate in group B was 100%. In group B, 13 cases were excellent, 6 cases were good, and 1 case was moderate, with an excellent and good rate of 95%. The postoperative complications were observed in group A, including avascular necrosis of lateral condyle in 1 case, premature closure of epiphysis in 1 case, skin irritation in 5 cases, needle exudation and incision infection in 2 cases, hyperplasia of lateral condyle in 3 cases, and loss of extension and flexion in 1 case.Conclusions There was no significant difference in operation time, elbow function recovery and imaging measurement between the two surgical methods in the treatment of lateral condylar fracture of humerus. Kirschner wire group had a certain advantage over cannulated cancellus screw group in fracture healing and removal of internal fixation. however, the clinical observation screw group has advantages in postoperative nursing and early exercise.
