Management of calcaneal fractures: systematic review of randomized trials

Abstract: There is still a need for a carefully designed large-scale trial comparing surgery and non-operative management. Other forms of fixation (external fixation or minimally invasive internal fixation) should be compared with 'conventional' surgery. Trials investigating joint reconstruction versus primary subtalar fusion for highly comminuted fractures, and impulse compression versus placebo could be of value.

“…However, the lateral approach has its disadvantages as it contributes to the development of local complications connected with difficult wound healing. According to the literature, this is a problem in 10-25% of the patients [12,13,22]. In our group, superficial wound infection was found only in 5 patients (7.04%), while 1 subject had skin necrosis requiring a graft.…”

“…Despite the high number of methods available, the treatment of calcaneal fractures remains a controversial issue and the long-term outcomes are varied and often unsatisfactory for the patient as well as the surgeon [22,23]. Even though many authors report that the best results are achieved with surgical treatment, some authors opt for conservative methods [1,24].…”
