Myasthenia gravis (MG) is a syndrome of fluctuating skeletal muscle weakness that worsens with use and improves with rest. Eye, facial, oropharyngeal, axial, and limb muscles may be involved in varying combinations and degrees of severity. Its etiology is heterogeneous, divided initially between those rare congenital myasthenic syndromes, which are genetic, and the bulk of MG, which is acquired and autoimmune. The autoimmune conditions are divided in turn between those that possess measurable serum acetylcholine receptor (AChR) antibodies and a smaller group that does not. The latter group includes those MG patients who have serum antibodies to muscle-specific tyrosine kinase (MuSK). Therapeutic considerations differ for early-onset MG, late-onset MG, and MG associated with the presence of a thymoma. Most MG patients can be treated effectively, but there is still a need for more specific immunological approaches.

A patient with invasive malignant thymoma and renal metastases was treated with cis-platinum, resulting in a complete response as documented by CTT and gallium scans. The association of renal metastases from thymoma is discussed.Cancer 48~223-226. 1981. w M i c TUMORS are the most common malignancyT found in the arterior mediastinum in adults.'O Extrathoracic spread of malignant thymoma is uncommon and the frequency is estimated to be about 6%.15 This paper reports the occurrence of renal metastases in a patient with thymoma and reviews the incidence of renal and other extrathoracic metastases in this disease. It also demonstrates a complete response to cisdiamminedichloride platinum I1 (cis-platinum). Case ReportA 19-year-old woman complained of left shoulder pain and hoarseness in December 1978. Left vocal cord paralysis was discovered and a chest radiograph revealed an anterior mediastinal mass. Mediastinoscopy and biopsy of the tumor revealed no abnormality, and she was referred to Memorial Hospital where a left thoracotomy was performed. An invasive tumor was found involving the anterior mediastinum, pericardium, left hilum, and phrenic nerve; this tumor was not resectable. The mediastinal tumor was an epithelial thymoma composed of solid sheets of small and slightly elongated cells ( Fig. 1) with an irregular and oval shaped vesicular nucleus and indistinct cytoplasmic borders (Fig. 2). Occasional mitotic figures and a moderate vascular stroma were seen. Cell-surface markers for lymphoma were negative in the material obtained from this tumor.The patient was treated with radiation therapy consisting of a total dose of 3600 rads over six weeks delivered to the mediastinum and both supraclavicular areas with a depth of 9 cm and a field size of 26.5 x 25 cm. An additional 300 rads was delivered to the mediastinum alone. A repeat chest radiograph suggested residual tumor in the left hilum; therefore in July, 1979, a second thoracotomy was done. Necrotic tissue with dense fibrosis and clusters of lymphocytes and plasma cells without identifiable viable tumor was found (Fig. 3).One month later she experienced low back pain and a single episode of gross painless hematuria. Cystoscopy was negative but urine cytology was positive for malignant epithelial cells (Fig. 4). An intravenous pyelogram showed a mass in the lower pole of the right kidney. A CTT scan of the abdomen revealed two homogenous masses in the right kidney (Fig. 5 ) , which on selective renal arteriography were hypovascular. A positive uptake in the right kidney and both inguinal regions was revealed in 67gallium citrate scan. Further evaluation consisting of whole lung tomography, upper gastrointestinal series, and spine films showed no abnormality. Aspiration biopsy of the right kidney revealed clusters of malignant epithelial cells with cytologic features similar to the original mediastinal thymoma (Fig. 6).The patient was treated with high-dose cis-platinum (120 mg/m2) and manitol infusion* at three-week intervals. A case of invasive thymoma with metastasis to liver and brain is presented. The incidence, sites of involvement, and the different histological types of metastatic lesions are discussed. Review of the literature shows that the important prognostic factors are the infiltrative nature, the extent of the tumor at the primary site, and associated syndromes like myasthenia gravis and red cell hypoplasia. Patients with noninvasive thymoma have excellent survival by surgery, compared to a reported low (23%) five-year survival with invasive type. The average time of survival after the appearance of metastasis is about one and one-half years.
