Malignant mixed mullerian tumors of the ovary: experience with cytoreductive surgery and platinum-based combination chemotherapy

Abstract: This study reviews the clinical outcome and prognosis of patients with malignant mixed müllerian tumors (MMMTs) of the ovary treated with optimal cytoreductive surgery, leaving no residual disease, and platinum-based chemotherapy. Ten patients diagnosed with MMMT of the ovary after complete surgical staging from February 1993 to February 2004 at Asan Medical Center in Korea were studied retrospectively. All ten patients were treated with optimal cytoreductive surgery, leaving no gross residual disease. Seven p… Show more

“…These tumors histologically consist of both carcinomatous and sarcomatous components. Ovarian carcinosarcomas are subclassified as heterologous or homologous, depending on the presence or absence of a stromal component containing mesenchymal tissue not normally found at the primary tumor site [2]. …”

“…In an early study, Sood et al showed that patients with cancers with homologous sarcomatous elements had a significantly better survival compared to those with cancers containing heterologous elements [9]. But recent studies have suggested that there was no correlation and currently sarcomatous type is not considered a prognostic factor [4]. On the other hand, serous epithelial components have been proven to be associated with a worse survival compared to nonserous epithelial components [10].…”

“…This was probably due to the small number of our series. Some investigators presumed CA-125 to be a useful marker and integrated this parameter into their studies [4,5]. However, other investigators did not consider the use of CA-125 in their patients [6,7].…”

“…5 Overall survival of patients with MMMT of the fallopian tube was found to be 16.1 months, in one review, and this survival time was similar to that estimated for the malignant mixed Müllerian tumors of the ovary. 6,8 Consequently, carcinosarcomas are highly aggressive tumors and despite all these therapies, their prognosis are still fairly poor. Hence, it was deemed necessary to practice new treatment regimens to improve the survival time.…”
