Objectives. To investigate the failure patterns and the prognostic factors following postoperative radiotherapy for salivary gland carcinoma. Design. Retrospective study. Setting. University teaching hospital, Hong Kong. Patients. Fifty patients who had non-disseminated salivary gland carcinoma and who received primary treatment from 1984 through 1993. Main outcome measures. Demographic data, cancer T-and N-stages, histological type, site of origin, completeness of surgery, whether postoperative radiotherapy was given, and the clinical outcome. Results. Two (4%) patients had been treated with radiotherapy alone, six (12%) had undergone radical resection alone, and 42 (84%) had been radically treated by using both modalities. The 5-year overall survival and relapse-free survival rates were 78.4% and 63.1%, respectively. The free from local failure and free from distant metastasis rates at 5 years were 77.2% and 72.8%, respectively. The N-stage was a significant prognostic factor. The site of the primary tumour, T-stage, completeness of surgery, and use of postoperative radiotherapy were not significant independent prognosticators; however, among the T-stage tumours, the b-substage carcinomas had significantly fewer local failures (P=0.040) and better survival rates (P=0.038) than the a-substage carcinomas. There were seven (14%) locoregional failures without distant metastasis, seven (14%) cases of distant metastasis without locoregional failures, and four (8%) locoregional failures preceding distant metastasis; isolated regional relapse was rare (1/50; 2%). All regional failures (5/50; 10%) occurred ipsilateral to the primary lesion. There were no deaths due to lymphoepithelioma-like carcinoma or acinic cell carcinoma. Conclusions. The N-stage is the main prognostic factor of overall survival, relapse-and metastasis-free recovery, and success of treatment for salivary gland carcinoma. Optimal locoregional treatment can help reduce distant metastasis, and the maximal use of postoperative radiotherapy may contribute to improved locoregional control. Elective ipsilateral neck radiotherapy is indicated for lymphoepithelioma-like carcinoma.

In this study, the correlations of factors with Epstein-Barr virus (EBV)-association were investigated in 50 patients with nasopharyngeal carcinoma (NPC), 61 with oropharyngeal carcinoma (OPC), and 55 with hypopharyngeal carcinoma (HPC) in Okinawa and Osaka prefectures in Japan. The incidence of pharyngeal carcinoma in Okinawa was previously found to be higher than that in Osaka; the incidence of OPC was approximately 6 times higher and that of HPC was two times higher. The EBV genome was detected in the tumor cells of the present patients; 83% of the Okinawa and 92% of the Osaka NPC patients. The EBV genome was not detected in OPC or HPC. A univariate analysis showed that sex, the location of the tumor, histology, and the degree of lymphocytic infiltration correlated with the EBV-positive rate. A multivariate analysis revealed that only the location of the tumor was independently correlated with the EBV-positive rate. Histology and tumor size were factors affecting the prognosis of the patients with NPC. The NPC of poorly differentiated type frequently showed the EBV genome, and NPC with lymphocytic infiltration showed a more favorable prognosis compared to the other NPC types. These findings suggest that latent genes of EBV expressed in cancer cells might trigger a cytotoxic T cell reaction against the cancer. Key words: Epstein-Barr virus -Pharyngeal carcinoma -EpidemiologyEpstein-Barr virus (EBV), a herpesvirus which can infect lymphocytes, was originally discovered in cultured lymphoblasts from the African type of Burkitt's lymphoma in 1964.1) An oncogenic role of EBV in the development of B cell lymphoma and nasopharyngeal carcinoma (NPC) was subsequently suggested.2) Recent studies have shown the presence of EBV sequences in carcinoma cells of thymic, 3) gastric, 4) salivary gland, 5,6) and lung cancers. 7)The pharyngeal space is divided into three parts; the nasopharynx, oropharynx, and hypopharynx. The nasopharynx is covered with pseudostratified respiratory epithelium, and the oropharynx and hypopharynx are covered with stratified squamous epithelium. The naso-and oropharynxes have well-developed lymphoid tissue, i.e., Waldeyer's ring, whereas the hypopharynx is devoid of lymphoid tissue. The association of EBV with NPC is well-known, and it has been reported that EBV-associated NPC usually have poorly differentiated morphology accompanied by severe lymphoid cell infiltration, forming a lymphoepitheliomatous lesion. Salivary gland tumors usually occur as single lesions. To have more than one tumor is unusual. We report a case of an adult male who presented with a mucoepidermoid carcinoma involving the minor salivary glands of the palate at age 57 years, followed by an adenoid cystic carcinoma of the floor of mouth at age 63 years. The patient later succumbed to non-Hodgkin lymphoma at age 72 years. There are 31 acceptable cases of multiple malignant salivary gland neoplasms reported in the world literature. Multiple malignant tumors of the same histologic type are more common than those of different histologic type. Bilateral acinic cell adenocarcinoma was the most frequent combination of multiple salivary gland malignancy, accounting for 14 cases (10 synchronous and four metachronous). All involved the parotid glands bilaterally with the exception of one case that involved parotid and submandibular gland. Polymorphous low-grade adenocarcinoma accounted for three of the four cases of multiple malignant tumors involving minor salivary glands. Individuals with a history of malignancy are at risk for the development of additional malignant tumors and should receive appropriate clinical follow-up.
