Malignant Hemangiopericytoma in the Pelvic Cavity Successfully Treated by Combined-Modality Therapy: Report of a Case

Abstract: A 55-year-old Japanese woman underwent extirpation of a malignant hemangiopericytoma in the pelvic cavity, followed by postoperative irradiation. An abdominal computed tomography scan 3 years later revealed a local recurrent tumor, 12 cm in diameter, in the pelvic cavity, for which transarterial embolization was done, followed by excision of the tumor employing Hartmann's procedure. Although an unresectable part of the recurrent tumor remained, postoperative irradiation reduced its size remarkably. The patient… Show more

“…Other pharmaceutical agents such as pazopanib, a tyrosine-kinase inhibitor, have exhibited clinical efficacy and the potential for management of metastatic hemangiopericytomas; however more studies still need to be conducted [ 20 ]. Reports regarding radiation therapy on the other hand have been inconsistent [ 16 , 21 24 ]. Our case was managed surgically with complete resection of the primary tumor and each of the two successive recurrences.…”

“…In essence, if the time interval between the primary lesion and the liver metastatic disease is prolonged, major hepatectomy is justified in young individuals. In case of liver recurrence, re-resection combined with TAE is highly recommended, due to the hypervascular nature of the tumor [ 20 ].…”

“…The first one is arterial embolisation through Seldinger technique. There are well suceeded cases of hepatic artery embolisarion which reduced tumor size allowing curative resection [8]. Portal vein embolisarion induces vicarious hypertrophy of the future liver remnant permitting resection in primarily unresectable patients due to the small liver remnant expected.…”
