Malignant Chondroblastoma of the Os Calcis

Abstract: Patient. We describe a case of chondroblastoma of the os calcis which metastasized to the tibia, soft tissues and lung. A complete response of the lung lesions was noted with chemotherapy.Discussion. Review of the published literature shows that metastatic chondroblastoma only arises following local recurrence of the tumour.

“…Recurrence rates of 10-40% have been described [1,5,9,13]. There have been rare reports of seeding into soft tissues locally, and of pulmonary metastasis in about 1% of cases [12,14]. Growth disturbance rates and growth deformities are not widely published in the available literature.…”

“…In pulmonary metastasis, thoracotomy has produced apparent long-term cures, while in other sites the lesions have remained static following simple biopsy [7,25,27]. The benefit from chemotherapy is debatable [6,25,27]. Some authors recommend that any patient with a local recurrence should be screened for metastases [13].…”

“…Metastasis only followed local recurrence in association with malignant transformation at the primary site [6,27]. Patients usually survive several years with metastatic lesions.…”

“…There have been several reported cases of multifocal chondroblastoma, which were considered as multiple primary lesions (12,13). Aggressive chondroblastoma with pulmonary metastasis is considered to be 'malignant' chondroblastoma (14). The term 'aggressive' chondroblastoma has been applied from a clinical point of view rather than as a histopathological entity.…”
