ÖzetGiriş: Van yöresinde nedeni belirlenemeyen primer infertilite olgularının semen kültürlerinde Ureaplasma urealyticum(UU) ve Mycoplasma hominis(MH) sıklığı ve tedavinin sperm parametrelerine etkisinin araştırılması amaçlandı. Gereç ve Yöntem: 106 olgunun dahil edildiği bu çalışmada, infertil gurup (41 olgu), alt üriner sistem semptomları olan gurup (33 olgu) ve kontrol gurubu (32 olgu) olmak üzere 3 gurup oluşturuldu. Çalışma gurubundaki infertil olguların özgeçmişlerinde varikosel, testis torsiyonu, hidrosel, inmemiş testis öyküsü ve hormonal nedenler yoktu. Kontrol gurubundaki olgular, infertil ve alt üriner sistem semptomu olmayan olgulardan oluşturuldu. İnfertil gurupta, kültür pozitif olguların semen parametreleri tedavi öncesi ve sonrası incelendi. Mycoplasma türlerinin ayırımı Biomerieux® Mycoplasma IST 2 (RCS Lyon-France) kitleriyle yapıldı. Sperm sayımı Makler sayım kamarası ile yapıldı (Self Medical Industries, Haifa, Israel). Bulgular: İnfertil guruptaki 17 olguda (% 41,5) UU, 3 olguda (% 7,5) MH izole edildi. AÜSS'lu gurupta UU izole edilen olgu sayısı 15 (%45,5), MH izole edilen olgu sayısı 3 (% 9,1) idi. Kontrol gurubundaki olgularda MH izole edilmezken, 6 olguda (% 18,8) UU izole edildi . İnfertil guruptaki kültür pozitif 3 (% 15) olgunun sperm sayısı < 15 milyon iken kültür negatif olgularda ise 10 olgunun (%50) sperm sayısı 15 milyonun altındaydı ( p< 0,025). Tedavi öncesi kültür pozitif 14 olgunun (% 70) sperm motilitesi % 40'nin altındaydı. Tedavi sonrasında ise 5 olgunun (% 25) sperm motilitesi < % 40 idi. Motilitede anlamlı artış olduğu izlendi. Tedavi sonrası, infertil guruptaki kültür pozitif 20 olgunun (% 49) 4'ünün (% 20) eşinde gebelik sağlandığı görüldü (p<0,025) .Tartışma: Antibiyotik tedavisinin nedeni belirlenmemiş kültür pozitif infertil olgularda sperm motilitesine olumlu etki yaparak tedaviye olumlu katkı sağladığı izlenmiştir. Material and Method: The study included 106 individuals divided into three groups: The infertile group (41 cases), the group with lower urinary tract symptoms (33 cases), and the control group (32 cases). The patients in the infertile group had no history of varicocele, testicular torsion, hydrocele, undescended testis, and hormonal disorders. The control group included cases without infertility and lower urinary tract symptoms. The parameters of culture-positive cases in the infertile group were determined before and after therapy. Although a high prevalence of ASA was reported in the presence of Chlamydia in genital secretion as detected by means of culture (63) or the polymerase chain reaction (64), circulating chlamydial antibodies were not associated (65) or only weakly associated (66) with the presence of sperm bound antibodies, whereas both a strong correlation (66) as well as no association (67) were reported between the presence of seminal chlamydial antibodies and ASA.
