Malacoplakia of the Vallecula Masquerading as a Malignancy

Abstract: Malacoplakia is an unusual chronic granulomatous infl ammation that commonly occurs in the urinary tract. It is rarely encountered in the head and neck region, and only a handful of such cases have been described. We report a case of malacoplakia in a 50-year-old woman who presented with a mass in the vallecula that was clinically suspected of being malignant. Th is case illustrates the importance of recognizing this unusual benign lesion, which closely mimics a malignancy on clinical examination.

“…Malakoplakias continue to be reported to occur at unusual sites, such as the spleen, nasal cavity, and oropharynx [ 16 18 ]. The exact frequency of the association of malakoplakia with malignancy is not known at present and whether all cases of malakoplakia are to be definitively managed with surgery due to this risk is open for deliberation.…”

“…Malakoplakia is a rare inflammatory disease process which often arises as soft, yellow or fleshcolored plaques or papules, typically within the genitourinary or gastrointestinal tracts [1]; however, this clinical presentation varies depending on the site of involvement [2]. Cutaneous lesions of malakoplakia present as ulcerated plaques or nodules [3,4] while visceral involvement more commonly forms masses and affects sites such as the abdomen or the head and neck [2,5]. Oral involvement is exceedingly rare [1,6,7].…”
