Magnetic resonance imaging of neurohypophyseal germinomas

Abstract: The authors reviewed magnetic resonance (MR) images in seven cases of germinoma in the hypothalamoneurohypophyseal axis (HNA). The intrasellar portions were clearly identified in six germinomas. Two small germinomas of these six were located only in the neurohypophysis. The major parts of the four large germinomas were located below the optic chiasm, and the large intrasellar portions were demonstrated. The remaining one small germinoma was localized from the pituitary stalk to the third ventricular floor. The… Show more

“…Thanks to magnetic resonance imaging (MRI), an early diagnosis of pituitary stalk infiltration with lack of posterior pituitary hyperintensity is now possible (2)(3)(4)(5). In particular, the presence of an isolated thickened pituitary stalk is a frequent finding with evolution to either germinomas (1,9) or a self-limiting disease as in lymphocytic infundibuloneurohypophysitis (3). Diagnosis of the cause of central diabetes insipidus remains a challenge in many cases.…”

“…3 Conversely, in germinomas, the PBS is noted to be absent in 100% of cases, whether the patient presents with DI or not. 9 Failure to demonstrate a PBS in the context of a sellar and suprasellar mass should therefore prompt the clinician to consider the possibility of a germinoma, as well as other diagnoses associated with this feature.…”

“…When DI was a presenting symptom of one of these conditions, the PBS was always absent. 7,9,12,15,17,18 In conditions in which the posterior pituitary is depleted of its vasopressin granules, the hyperintense T1 signal can also be lost. An absent PBS was observed in 32% of patients with uncontrolled diabetes mellitus 11 and in a patient with severe anorexia nervosa.…”

“…It should be assumed that a child or adolescent with acquired DI harbors a suprasellar germinoma until proven otherwise [1,2]. Modern noninvasive and more sensitive diagnostic procedures allow earlier detection of the tumor [1,3]. In this MRI era, however, it has to date been unanswered when and how frequently the examination should be carried out for young patients with DI whose first MRI is normal.…”
