Abstract Background Postmastectomy secondary lymphedema can cause substantial morbidity. However, few studies have investigated longitudinal quality of life (QoL) outcomes in patients with postmastectomy lymphedema, especially with regard to surgical versus nonoperative management. This study prospectively investigated QoL in surgically versus nonsurgically managed patients with postmastectomy upper extremity lymphedema. Methods This was a longitudinal cohort study of breast cancer-related lymphedema patients at a single institution, between February 2017 and January 2020. Lymphedema Quality of Life Instrument (LyQLI) and RAND-36 QoL instrument were used. Mann–Whitney U and Fisher's exact tests were used for descriptive statistics. Wilcoxon's signed-rank testing and linear modeling were used to analyze longitudinal changes in QoL. Results Thirty-two lymphedema patients were recruited to the study (20 surgical and 12 nonsurgical). Surgical and nonsurgical cohorts did not significantly differ in clinical/demographic characteristics or baseline QoL scores, but at the 12-month time point surgical patients had significantly greater LyQLI overall health scores than nonsurgical patients (79.3 vs. 58.3, p = 0.02), as well as higher composite RAND-36 physical (68.5 vs. 38.3, p = 0.04), and mental (77.0 vs. 52.7, p = 0.02) scores. Furthermore, LyQLI overall health scores significantly improved over time in surgical patients (60.0 at baseline vs. 79.3 at 12 months, p = 0.04). Besides surgical treatment, race, and age were also found to significantly impact QoL on multivariable analysis. Conclusion Our results suggest that when compared with nonoperative management, surgery improved QoL for chronic, secondary upper extremity lymphedema patients within 12-month postoperatively. Our results also suggested that insurance status may have influenced decisions to undergo lymphedema surgery. Further study is needed to investigate the various sociodemographic factors that were also found to impact QoL outcomes in these lymphedema patients.

Background: The aim of this study was to validate a translated Swedish version of the lymphoedemaspecific quality of life questionnaire (LYMQOL) in a cohort of Swedish cancer patients with secondary lymphoedema of the limbs after cancer treatment. Material and methods: We recruited 102 patients with lymphoedema of the arms or legs after cancer treatment who were visiting lymphoedema therapists at the departments of oncology at the university hospitals in Link€ oping and Umeå. The LYMQOL questionnaires were translated forward and backward from English to Swedish. Content and face validity were evaluated. The construct validity was assessed by comparing the LYMQOL with the Short Form Health Survey (SF-36) and the perceived degree of lymphoedema of the limbs, respectively. Reliability was determined through test-retest. The internal consistency was assessed by determining Cronbach's alpha and by factor analysis. Results: The content and face validity assessments showed that LYMQOL was an easy, clear and not too long questionnaire to use for patients with lymphoedema. Construct validity was high in both versions when compared with the SF-36. The association between the degrees of perceived lymphoedema and the LYMQOL was only significant in the domains Function and Body Image in the arm version, whereas all domains in the leg version were significant. The reliability was good for the arm version (intra-class-correlation coefficients 0.53-0.87) and very good for the leg version (intra-class-correlation coefficients 0.78-0.90). The internal consistency was acceptable to excellent, with Cronbach's alpha values between 0.79-0.93 (arm-version) and 0.87-0.94 (leg-version). The factor analysis confirmed the usefulness of the four domains in the LYMQOL versions. Conclusions: This study confirmed the validity of the Swedish version of LYMQOL and demonstrated that LYMQOL may be a simple and useful tool for use in clinical practice and scientific contexts for evaluating QoL in patients with lymphoedema of the limbs.

Bu çalışmada meme kanseri tedavisi ile ilişkili hafif, orta ve şiddetli lenfödemi olan hastalar arasındaki yaşam kalitesi, üst ekstremitenin fonksiyonel durumu ve fiziksel aktivite düzeyi farklılıklarını karşılaştırmak amaçlanmıştır. Gereç ve Yöntemler: Çalışmaya meme kanseri tedavisi ile ilişkili lenfödem tanısı olan toplam 83 kadın hasta (%24 hafif, %41 orta, %35 şiddetli) alındı. Lenfödemin varlığı ve şiddeti çevre ölçümü ile belirlendi. Lenfödem Yaşam Kalite Ölçeği (LYKÖ); Kol, Omuz ve El Sorunları Anketi'nin (KO-ESA) kısa versiyonu; ve Uluslararası Fiziksel Aktivite Anketi'nin kısa formu (UFAA-KF), sırasıyla yaşam kalitesini, üst ekstremitenin fonksiyonel durumunu ve fiziksel aktivite düzeyini değerlendirmek için kullanıldı. Bulgular: Hafif, orta ve şiddetli lenfödemi olan hastalar arasında yaşam kalitesi ve üst ekstremitenin fonksiyonel durumu açısından istatistiksel olarak anlamlı fark vardı (p<0,05). Ancak fiziksel aktivite düzeyi bakımından üç grup arasında anlamlı fark yoktu (p>0,05). İkili analizlerin sonuçlarına göre, şiddetli lenfödemi olan hastalarda, yaşam kalitesinin fonksiyon, görünüm ve semptom alt alanlarının ve üst ekstremitenin fonksiyonel durumunun hafif derecede lenfödemi olan hastalara göre daha fazla etkilendiği bulundu (p<0,016). Lenfödem şiddeti ile LYKÖ fonksiyon (r=0,34), görünüm (r=0,32) ve semptom (r=0,30) alt alanları ve KOESA skorları (r=0,33) arasında istatistiksel olarak anlamlı ancak zayıf bir ilişki bulundu (p≤0,01). Tartışma ve Sonuç: Şiddetli lenfödemi olan hastalarda yaşam kalitesinin ve üst ekstremitenin fonksiyonel durumunun hafif lenfödemi olan hastalara göre daha fazla etkilendiği bulundu. Bu sonuçlar, meme kanseri cerrahisi geçiren bireylerde lenfödem şiddeti hafif düzeyden ileri düzeye geçtiğinde yaşam kalitesinin ve üst ekstremite fonksiyonelliğinin etkilenebileceğini göstermektedir. Dolayısıyla bu etkiyi önlemek veya azaltmak için lenfödemin erken teşhis ve tedavisi önemlidir.
