The objective was to prepare guidelines to perform the current optimum treatment by organizing effective and efficient treatments of hemangiomas and vascular malformations, confirming the safety, and systematizing treatment, employing evidence‐based medicine (EBM) techniques and aimed at improvement of the outcomes. Clinical questions (CQs) were decided based on the important clinical issues. For document retrieval, key words for literature searches were set for each CQ and literature published from 1980 to the end of September 2014 was searched in Pubmed, Cochrane Library, and Japana Centra Revuo Medicina (JCRM). The strengths of evidence and recommendations acquired by systematic reviews were determined following the Medical Information Network Distribution System (MINDS) technique. A total of 33 CQs were used to compile recommendations and the subjects included efficacy of resection, sclerotherapy/embolization, drug therapy, laser therapy, radiotherapy, and other conservative treatment, differences in appropriate treatment due to the location of lesions and among symptoms, appropriate timing of treatment and tests, and pathological diagnosis deciding the diagnosis. Thus, the Japanese Clinical Practice Guidelines for Vascular Anomalies 2017 have been prepared as the evidence‐based guidelines for the management of vascular anomalies.

38Lenfanjiyoma Sirkumskriptum:Üç Çocuk Olgu Ö ÖZ ZE ET T Lenfanjiyoma sirkumskriptum doğumda mevcut olabilen veya doğumdan hemen sonra ortaya çıkabilen, deride lokalize alanlarda berrak veziküller ile karakterize, nadir görülen vasküler hamartomatöz bir durumdur. Lezyonlar çoğunlukla kurbağa yumurtasını andırır şekilde küme oluşturmuş ve herpetiform yapıdadır. 3-4 mm çaplı vezikül kümecikleri berrak olabildiği gibi, pembe, kırmızı veya siyah renkte kanama içerebilmektedir. Genellikle alt ekstremitenin proksimal kısımları, kalça, kasık, aksiller kıvrım, boyun ve özellikle dilde lezyonlar görülebilmektedir. Doğumsal lezyonların yanında edinsel olarak da ortaya çıkabilmektedir. Bu şekilde bildirilmiş vakaların lokalizasyonu genellikle vulvar ve perianal yerleşimlidir ve cerrahi girişimler, radyoterapi ve enfeksiyonlar sonrası gelişebilmektedir. Bu çaluışmada, farklı lokalizasyonda, farklı klinik progresyonlar gösteren üç doğumsal Lenfanjiyoma sirkumskriptum olgusu bildirilmişdir.A An na ah h t ta ar r K Ke e l li i m me e l le er r: : Lenfanjiyoma; lenfanjiyom,kistik A AB BS S T TR RA AC CT T Lymphangioma circumscriptum is a rare hamartomatous condition characterized by clear vesicles which can be present at birth or soon after in localized areas of the skin. Lesions are often reminiscent of the frog spawn, grouped and in a herpetiform pattern. As vesicles of 3-4 mm in diameter may be clear, they consist of pink, red, black haemorrhage. The most common locations are proximal aspect of the limbs, buttock, groin, axillary folds,neck and oral mucosa especially the tongue. In addition to congenital lesions, acquired lesions may also occur. Such lesions are usually located vulvar and perianal area and can be seen after surgical procedures, radiation therapy and infection. Résumé -Introduction : Les malformations lymphatiques (ML) sont congénitales et non régressives. Elles résul-tent d'une anomalie de la morphogenèse embryologique vasculaire et sont détectées dans 80 % des cas avant l'âge de 2 ans, exceptionnellement à l'âge adulte. Asymptomatiques, elles sont le plus souvent diagnostiquées suite à une gêne fonctionnelle. Bien que d'aspects cliniques très hétérogènes, elles se regroupent en deux formes cliniques à distinguer, macrokystiques et microkystiques, dont les traitements et les pronostics diffèrent. L'aspect clinique de la lésion et la symptomatologie sont essentiels pour l'établissement du diagnostic mais aussi pour la décision thérapeutique. Bien qu'une surveillance clinique régulière soit souvent suffisante, une intervention thérapeutique est parfois nécessaire. Observations cliniques : Quatre cas de ML orales avec des aspects cliniques très hétéro-gènes sont présentés. L'un est situé au niveau de l'espace masticateur et de la fosse infratemporale avec une expression muqueuse à la commissure intermaxillaire, deux au niveau des muqueuses jugales et labiales, le quatrième au niveau de la langue. Discussion : Pour trois d'entre eux, le diagnostic a été réalisé à l'âge adulte et ceuxci ne présentaient aucun antécédent de ML. Deux ont été traités par laser, un par chirurgie, le dernier a bénéficié d'une surveillance clinique régulière. Conclusion : Ces quatre cas cliniques sont particulièrement représentatifs de l'hétérogénéité clinique buccale et thérapeutique de ces malformations.Abstract -Oral lymphatic malformations, case reports and review of the literature. Introduction: Lymphatic malformations (LMs) are congenital and non-regressive lesions. These lesions are due to embryological vascular morphogenesis abnormality. Most of them (80 %) are detected before 2 years old, and thus they are rarely found in adults. They are asymptomatic, so their diagnosis is often linked to functional discomfort. There are two clinical aspects with different prognoses: macrocystic and microcystic. Clinical aspects and symptomatology are essential for positive diagnosis and therapeutic decisions. For most cases, simple clinical observation is advised. However, in other cases associated with aesthetic or functional discomfort, therapeutic intervention is recommended, most of the time with sclerotherapy or surgery. Clinical observation: We present four patients with different types of lymphatic malformation: one is on the intermaxillary commissure, the second is on the cheek mucosa, the third is on the lip and the fourth is on the tongue. Discussion: For three of them, the diagnosis was established in adulthood with no past history of LMs. Two patients were treated by laser, one with surgery and for the last one, clinical follow-up was established. Conclusion: These four case reports are representative oral illustrations of the clinical aspects and therapeutic heterogeneity of this pathology.
