Lung cancer presenting with trigeminal neuropathy

Abstract: The purpose of this case report is to describe MR imaging and CT findings of a patient in whom the first clinical and neuroradiological manifestation of a metastatic lung adenocarcinoma was unilateral trigeminal neuropathy caused by a presumed metastasis involving cisternal and Meckel's cave segments of ipsilateral trigeminal nerve. MR imaging and CT scan differential diagnosis of expansive lesions of the intracranial trigeminal nerve must include metastases. Physicians and neuroradiologists must be aware of m… Show more

“…A common finding in this pathology is enlargement of the ganglion with splaying of the nerve fibers. Metastasis of pulmonary adenocarcinoma to the DRG has not previously been described but cases of trigeminal ganglion metastasis have [4]. Our case report provides further information on the potential of this entity to spread to the border between the central and peripheral nerve systems.…”

“…Eight (75%) of the 12 patients showed symptoms relating to sphenoid bone metastasis as an initial presentation of the lung carcinoma. Only one report described trigeminal neuropathy caused by metastasis to Meckel's cave, site of the ganglion of the trigeminal nerve, resulting in the symptoms seen as the first presentation of lung adenocarcinoma [8]. The reason our patient did not complain of any respiratory symptoms, back pain, or general symptom remains unclear.…”

“…Characteristics of 12 patients with metastasis of lung carcinoma to the sphenoid bone identified from the literature published since 1990 are summarized in the Table 1 [4, 5, 6, 7, 8, 9, 10, 11]. Median age of the patients was 63 years (range, 47–76 years).…”

“…Only a handful of cases have ever been reported in the literature with primary malignancy sites including breast, colon, kidney, prostate, lymphocytic, thyroid, esophagus, cervix, and endometrium. ( Hooker et al, 2018 , Soni, 2010 , Reshko, 2018 , Cerase, 2011 , Wang et al, 2016 , Sudha et al, 2019 ). Here we describe a report of a young woman with metastatic EOC to the femur, vertebrae, and Meckel’s cave with leptomeningeal spread, all found around the time of initial diagnosis.…”
