Purpose of reviewTo summarize the current understanding of rheumatoid arthritis-associated interstitial lung disease (RA-ILD) throughout the rheumatoid arthritis (RA) disease course from preclinical to established disease. Recent findingsILD is a serious extra-articular manifestation of RA. Multiple studies have demonstrated a high prevalence of both subclinical and clinical ILD throughout the RA disease course. Investigations of patients without RA have demonstrated an association between RA-related autoantibodies like anticitrullinated protein antibodies (ACPA) and interstitial abnormalities on lung imaging. A significant proportion of RA-ILD patients develop ILD prior to articular manifestations, suggesting that the lung plays a central role in RA development, perhaps through ACPA production. RA-ILD also occurs in early RA, when exuberant autoantibody production and systemic inflammation may propagate pulmonary disease activity. In patients with established RA, a high burden of subclinical and clinical ILD results in significant morbidity, mortality, and healthcare expenditure. Multiple epidemiologic and genetic risk factors, as well as serum biomarkers, have been associated with RA-ILD.

Introduction. Rheumatoid arthritis (RA) affects not only the joints but also other organs. Lung disease is one of the leading manifestations of RA, and detection of these changes remains a diagnostic challenge. Delayed diagnosis of pulmonary involvement in the pathogenesis of RA often leads to development of severe forms which aggravate prognosis and decrease of the quality of life. Identify the characteristics and prognostic significance of CT and SPECT symptoms involving respiratory system in patients with RA depending on the stage of the disease and the destruction of hand joints as detected by MRI. Methods. To evaluate favorable and unfavorable predictors of RA clinical course and detect changes in the lung using modern methods of imaging: magnetic resonance, computed tomography and single-photon emission computed tomography. Results. A set of signs identified by CT and SPECT was analyzed corresponding to the presence of different types of erosions detected by MRI. Pleural adhesions, symptoms of bronchial obstruction (uneven ventilation, «air trap»), and signs of vasculitis were observed in all stages of the disease. With the progression of the disease, bullous emphysema-type deforming bronchitis symptoms and associated comorbid processes in the lung tissue were observed. Conclusions. For all types of active RA in patients with a high frequency revealed signs of bronchial obstruction and concomitant manifestations of vasculitis. These results require the inclusion of X-ray and single photon emission computed tomography of the chest in the algorithm of diagnostic evaluation of the patients with RA to identify and assess changes related primarily to the blood circulation, have prognostic significance and impact on treatment regimen.

Rheumatoid arthritis is an inflammatory rheumatic disease with unknown etiology characterized by symmetric, chronic and erosive arthritis (synovitis) of the peripheral joints and systemic inflammatory involvement of the viscera. Lung pathology, including interstitial lung disease, is one of the common extra-articular manifestations at the гheumatoid arthritis. The leading diagnosis method of interstitial lung disease is a computed tomography high resolution, which allows to identifying interstitial lung changes in 70% of the cases. The study of prognosis in patients with rheumatoid arthritis interstitial lung disease has been the subject of several studies in the past decade. In most studies it was stated that the average life expectancy from diagnosis is around 3 years. This work is devoted to study of interstitial lung disease as one of the most frequent extra-articular ma-nifestations of rheumatoid arthritis. The authors defined the significance of computed tomography high resolu-tion for the diagnosis of pulmonary interstitial lung changes at rheumatoid arthritis. They give comparative clini-cal and immunological, instrumental characteristics of rheumatoid arthritis patients with interstitial lung disease and without it. The role of smoking and positivity of anti-cyclic citrullinated peptide (anti-CCP) according to antibodies are determined as risk factors of interstitial lung disease development in patients with rheumatoid arthritis. The interrelation of the X-ray tomography symptom of interstitial lung disease "ground glass opacity" with the activity and duration of rheumatoid arthritis is revealed. "Ground glass opacity" symptom is associated with high index of DAS28 in patients with rheumatoid arthritis with interstitial lung disease.
