Low risk of bacteremia after endoscopic variceal therapy for esophageal varices: a systematic review and meta-analysis

Abstract: Background and study aims: Endoscopic variceal ligation (EVL) and endoscopic variceal sclerotherapy (EVS) are the main therapeutic procedures for the emergency treatment and secondary prophylaxis of esophageal varices in cirrhotics. Post-endoscopic bacteremia has been reported after EVS and EVL, but data on the frequency of bacteremia are conflicting. This study aims to provide incidences of bacteremia after EVS and EVL in different settings through meta-analysis. Methods: Only prospective or randomized studie… Show more

“…We recently published a meta-analysis on bacteremia after both EVL and EVS and found only limited data on bacteremia after EVL ( 24 ). The data in this study had not yet been reported and so was not included in the meta-analysis.…”

“…La frecuencia de bacteriemia en pacientes sometidos a escleroterapia fue mayor (17%) comaprada con la observada (6%) en pacientes tratados mediante ligadura (p = 0.106). La incidencia de bacteriemia es baja en pacientes con cirrosis sometidos a escleroterapia o ligadura, sin sangrado activo, por lo que no se recomienda profilaxis con antibióticos 16 .…”

“…The rationale behind prophylaxis is the risk of bacteremia, which occurs in 3%-6% of cases, but it becomes clinically relevant only in a minority. A recently published systematic review and meta-analysis investigated this topic including 1001 procedures in 587 patients from 19 studies[ 38 ]. Overall, the frequency of bacteremia was 17% and 6% after sclerosis and band ligation, respectively.…”
