Low plasma aldosterone despite normal plasma renin activity in uncomplicated type 1 diabetes mellitus: effects of RAAS stimulation

Abstract: Plasma aldosterone was deceased relative to PRA in uncomplicated type 1 diabetic patients, irrespective sodium intake. The responsiveness to sodium restriction was adequate and sodium restriction was able to overcome the decreased plasma aldosterone response to exogenous AngI, which was observed during liberal sodium in diabetic patients. The lower aldosterone is not secondary to diabetic complications and does not depend on the level of sodium intake.

“…Another study also showed increased components of the reninangiotensin-aldosterone system in patients with diabetes mellitus type 1, namely increased plasma aldosterone levels ( Hollenberg et al, 2004 ). Other human studies indicate reduced aldosterone levels in type 1 diabetes mellitus ( Luik et al, 2003 ;Cronin et al, 1995 ). In a model of type 1 diabetes, the streptozotocin-diabetic rat, cardiac fi brosis was enhanced and could be attenuated by treatment with spironolactone ( Miric et al, 2001 ), suggesting an important role of aldosterone in diabetes mellitus.…”

“…The therapy for diabetes did not affect the change in PRA and ARR. N. K. Hollenberg and colleagues, in their work indicate that patients with diabetes mellitus show an increase in PRA, angiotensin II concentration and plasma aldosterone concentration (41). In the opinion of other authors, most patients with type 2 diabetes have normal circulating levels of plasma aldosterone.…”

“…N.K. Hollenberg i saradnici u svom radu iznose da pacijenti sa dijabetes melitusom pokazuju porast PRA, koncentracije angiotenzina II i plazma koncentracije aldosterona (41). Prema mišljenju drugih autora, ve ina pacijenata sa tipom 2 dijabetesa ima normalne cirkulišu e nivoe plazma aldosterona.…”

“…After completion of the baseline clearance period of 50 min, AngII (Clinalfa AG, Switzerland) was administered intravenously in a dosage of 8 g/kg per min for 60 min. This dosage is known to cause a moderate systemic and renal hemodynamic pressure response, as described previously (23). This was followed by a 20-min recovery period.…”
