Background: Acne vulgaris is a chronic inflammatory disease of pilosebaceous units. Oral isotretinoin is recommended for patients with moderate-to-severe acne vulgaris who are not responding satisfactorily to conventional therapies. Recent reports indicate that acne patients have been benefiting from the low-dose treatment protocols. Aims and Objectives: The aim of this study was to assess and compare the efficacy of oral isotretinoin in low-dose continuous and intermittent treatment of moderate-to-severe acne vulgaris. Materials and Methods: This was a prospective, randomized, open-labeled, comparative, efficacy study carried out at Outpatient Clinic in the Department of Dermatology in Mandya Institute of Medical Sciences, Mandya. Out of 120 patients screened, 100 patients were selected. Patients with moderate-to-severe acne were assigned equally (50 patients each) to one of the two treatment regimens using block randomization technique, Group A was given lowdose continuous regimen -20 mg of oral isotretinoin once daily for 4 months and Group B was given low-dose intermittent regimen -20 mg of oral isotretinoin once daily for 1 week out of every 4 weeks. The patients were followed up every 4 th week during the treatment period. A 6-month follow-up evaluation after the end of treatment was performed. The outcome of the therapy was based on the improvement in the global acne grading system (GAGS) score and patients' satisfaction with the treatment in the 4-point scale. Results: This study showed statistically significant clinical improvement and difference in GAGS score between Group A and Group B (P < 0.005). Conclusion: This study suggests that, considering clinical efficacy, low-dose continuous treatment is most suitable for patients with moderate-to-severe acne vulgaris.

<p>Acne é uma doença inflamatória crônica da unidade pilossebácea. Está entre as patologias dermatológicas mais comuns, afetando a população em qualquer fase da sua vida. O tratamento mais utilizado para a acne de graus moderado a grave é feito através da isotretinoína oral. A isotretinoína é um tipo de retinóide, quimicamente conhecida como ácido 13-cisretinóico. Os retinóides atuam no crescimento e diferenciação das células epidérmicas, assim como interferem na atividade da glândula sebácea e possuem propriedades imunomoduladoras e anti-inflamatórias. A forma mais difundida de retinóides atualmente é a isotretinoína. Esse retinóide apresenta forte potencial xerostômico, e sua administração sistêmica leva ao ressecamento de diversas estruturas orais e não orais. Este trabalho tem como objetivo apresentar uma breve revisão de literatura sobre o uso da isotretinoína no tratamento da acne e verificar como essa medicação pode levar à halitose. Foi realizada revisão bibliográfica na base de dados LILACS, SciELO, Pub Med, google acadêmico, bem como livros da área dermatológica e selecionado artigos e, outros matérias relevantes foram utilizados para a pesquisa.</p>

The aim of the present study was the evaluation of serum level of lipids and gamma glutamyl transpeptidase (GGT) activities in patients with acne vulgaris undergoing isotretinoin treatment. Fortyfour patients, with severe acne or moderate acne (age mean 23 6±6.0 year) who were resistant to conventional treatment were supplemented with 0.5 mg/kg/day of oral isotretinoin for 120 days. This study revealed a significant increase in serum level of cholesterol, triglyceride and GGT activity. The findings of this work showed a number of negative effects of isotretinoin on disturbances in lipid profile and antioxidant status, in acne patients. Therefore, to control abnormalities in lipid profiles and oxidative stress, we recommend further studies to use antioxidant such as vitamin E supplementation in patients using isotertinoin.
