Long-term visual acuity and the duration of macular detachment: findings from a prospective population-based study

Abstract: Our study suggests that the majority of patients with macula-off RRD successfully repaired with one operation will achieve a VA of 6/18 or better at final follow-up. After 8 days of macular detachment, the final visual outcome may be adversely affected and, thus, operative repair within this period is desirable. Duration of macular detachment of ≤8 days demonstrated a continuing improvement in VA for up to 1 year, a finding which was not found in macula detachments of longer duration.

“…But there was a threshold after 4 weeks when visual acuity further decreased. This is in line with other data [16] : thresholds for deterioration of results at 8 and 21 days [2] or after 10 days and 6 weeks of symptom duration were described [6] .…”

“…For example, Hassan et al [6] could find no effect on the final visual outcome for the first 10 days of duration of the detachment, but a decrease in visual acuity after 10 days and again after 6 weeks of symptom duration. Another study defined days 8 and 21 after the onset of symptoms as important thresholds that affect the visual outcome adversely [2] . In both studies, onset of symptom duration was equated to onset of retinal detachment.…”

“…1 Improvements in surgical techniques have resulted in an increased anatomical success rate; however, functional recovery is often compromised and some patients are not satisfied with the quality of life after surgery. 2,3 In macula-off RRD, visual distortion is a major complaint and postoperative metamorphopsia is a highly prevalent problem that occurs in around twothirds of patients. 4,5 M-Chart is a convenient and reliable method to evaluate metamorphopsia, and it has been used to assess the degree of visual distortion for macular pucker and macular holes in previous studies.…”

“…5 Persistent functional damage to the macula is observed in a proportion of eyes affected by macula-off RRD. [6][7][8][9][10][11] Factors reportedly associated with functional recovery following macula-off RRD include preoperative VA, 12 duration of macular detachment, 6,8 height of macular detachment, 13,14 and age. 12 Even among eyes predicted to have good postoperative vision due to good preoperative conditions and the absence of significant complications during surgery, some eyes continue to have poor VA.…”
