Long-term risk of recurrent stroke after a first-ever stroke. The Oxfordshire Community Stroke Project.

Abstract: Background and PurposeThere have been few communitybased studies of long-term prognosis after acute stroke. This study aims to provide precise estimates of the absolute and relative risks of stroke recurrence in an unselected cohort of patients with a first-ever stroke. Methods Six hundred seventy-five patients were registered in a community-based stroke register (the Oxfordshire Community Stroke Project) and prospectively followed for up to 6.5 years. Their relative risk of recurrent stroke was calculated usin… Show more

“…7,8 Many patients who could benefit from antiplatelet, 9 anticoagulant, 10 or antihypertensive 11,12 medication do not receive it. Although stroke patients are at a high risk of further vascular events 13 and may benefit from secondary prevention, population-based data on their vascular risk management is limited, and most of the available studies comprise hospital-based cohorts. 14 -16 In England and Wales the prevention of vascular disease has been identified as a priority.…”

“…15 Long-term risk of recurrent stroke after a firstever nonfatal stroke in the general population has been estimated at 30% actuarial risk by 5 years (9-fold increased risk vs nonstroke general population), with highest risk within the first year. 16 The decreased trend for stroke mortality after kidney transplant in our study cohort of stroke patients may reflect aggressive work up pretransplant to determine medical suitability and attenuation of cardiometabolic risk factors. However, this cannot be the complete explanation, as cardiovascular-related 55 Variables included in the model were age, gender, donor type (living vs deceased), socioeconomic deprivation, ethnicity, year of transplant, and selected medical comorbidities (ie, history of myocardial infarction, peripheral vascular disease, stroke, congestive cardiac failure pulmonary disease, liver disease, peptic ulcer, previous cancer and diabetes).…”
