Long-term results of percutaneous balloon valvuloplasty of congenital aortic stenosis in adolescents and young adults

Abstract: There is no statistical difference in the long-term outcome in the adults and adolescents as compared to the children; thus BAV remains an obvious treatment of choice with good long-term outcome.

“…Subsequently, a number of cardiologists have applied this technique and reported their results, extensively referenced elsewhere 4, 5, 8, 9, 10, 11, 12, 13. In this issue of the Journal, Awasthy and associates 14 evaluate long-term results of BAV with particular attention to adolescent and adults. They compared the outcomes of this subset with those of children below 1 year of age and those between 1 and 11 years.…”

“…Valvulotomy can be done surgically or percutaneously. Open valvulotomy is not common practice in the current era for young adults and was previously considered a palliative treatment, with reoperation needed in 25–40% of patients with in 10 years ( 52 54 ). Balloon valvuloplasty can be a safe and effective treatment for children presenting with congenital AS.…”

“…Balloon valvuloplasty can be a safe and effective treatment for children presenting with congenital AS. It confers palliative benefits by providing a reduction in gradient ranging from 49–70%, thus delaying the time for more definite surgical intervention ( 53 , 54 ). Currently this is less appealing compared to other interventions as there is a significant risk of developing severe aortic incompetence and there is a need for repeat balloon valvulotomy or definitive intervention.…”

“…Furthermore, most of the data regarding outcomes of BAV are obtained from studies performed in developed countries. Data regarding the experience and management of congenital aortic stenosis in developing countries are scarce [ 8 11 ].…”
