Long-term results of endoscopic third ventriculostomy: an outcome analysis

Abstract: T he technique of endoscopic third ventriculostomy (ETV) has been under close evaluation as a treatment option for obstructive hydrocephalus. Although early on the procedure was associated with a very high, unacceptable complication rate, 41 more recent publications have reported success rates of about 70% and low complication rates, including mortality, around 0.5%-1.0%. 9,13,20,31,35 Nowadays, ETV is accepted as the gold standard for treatment of obstructive hydrocephalus. In comparison with ventriculoperito… Show more

“…Our ETV success outcomes fall within the range of outcomes that have been previously reported by other centers. 1,13,18,29,33,36 Some of the highest efficacy data were presented by Woodworth et al, who observed immediate improvements in 89% versus 71% of patients who underwent primary and secondary ETV, respectively. 35 In a larger cohort of 129 patients, Jenkinson et al observed success rates of 83% and 67% in primary and secondary ETV groups, respectively.…”

“…44,45 Furthermore, as has been extensively reported in the literature on initial ETV, we found that 64% of failures occurred within the first 6 months after re-ETV. 21,26,35,42,53,55,63 Another reason for choosing ETV over shunt treatment is the apparent difference in concern among the parents of children with hydrocephalus. 40 Although the comfort of parents could be a reason to prefer ETV over shunting, it can also make parents less cautious (i.e., provide a false sense of security), as there is no certainty of a cure for hydrocephalus, and late failure of ETV does occur with potential devastating results.…”

“…Nevertheless, with regard to the managements of normal pressure hydrocephalus with ETV, there is no global consensus in the efficiency such method {Balevi:2017hj}{Tudor:2015hd}. As a result, the success rate of ETV varies from an overall 70 5 to 97% in the first year post-ETV, 6 while the complication rate remains low. 7 In our center, we had complications in five patients.…”
