Long-Term Outcomes of 23-Gauge Pars Plana Vitrectomy With Internal Limiting Membrane Peeling and Gas Tamponade for Myopic Traction Maculopathy

Abstract: Microincisional 23-gauge pars plana vitrectomy with ILM peeling and gas tamponade is effective in the treatment of myopic traction maculopathy, with low postoperative complications. Globally, both BCVA and macular thickness improved significantly during the follow-up period. However, greater visual acuity improvements were only seen in eyes with preoperative BCVA equal to or better than 20/63 Snellen equivalent. Some concerns remain about the risk of macular hole formation after ILM peeling. Further studies ar… Show more

“…Once the tractional component is removed, interstitial fluid may be reabsorbed by the RPE pump and the Müller Cells, with the slow restoration of the macular microstructure. [23][24][25] We speculate that the content of the intraretinal cystoid spaces may differ in cases of exudative versus tractional etiology. In the exudative subgroup the blood-retinal barriers are compromised and the intraretinal fluid may assume the characteristic of an exudate.…”

“…The inverted ILM flap approach remains technically challenging even for an experienced surgeon [90]. To further steady the flap in an adequate position, the vitreous cavity can be filled with gas or silicone oil [20, 26, 75].…”

“…The only prospective nonrandomized multicentre study for myopic foveoschisis, has confirmed a beneficial effect of vitrectomy and ILM peeling with gas tamponade on the reduction of central macular thickness, although the fovea was not spared during the ILM peeling in this series, and one patient developed a macular hole during the follow-up [11].…”
