Long‐term outcome of kidney transplant using non‐heart‐beating donor: multicenter analysis of factors affecting graft survival

Abstract: In the cases using NHBD, graft survival for 1, 5, and 10 yr was 87, 69, and 53%, respectively. Donor age of over 55 yr, acute early and late rejection, post-transplant HT and diabetes at the first post-operative year were shown to be significantly harmful on long-term graft survival. For longer graft survival in NHBD kidney transplantations, reducing acute rejection, and controlling blood pressure and sugar are crucial.

“…In a series of 242 kidney transplants, Mizutani et al [10] reported lower graft survival for recipients of kidneys aged >60 years compared with the younger uDCD donor group. These results were confirmed in a retrospective study of 706 kidney transplants from uDCD donors, where donor age >55 years had a negative impact on long-term graft survival [11]. In one of the largest series published, S anchez-Fructuoso et al [13] described that 1and 5-year graft survival for kidneys from uDCD donors with a maximum age of 60 years was similar to that of kidneys from DBD donors <60 and significantly better than that of kidneys from DBD donors ≥60 years.…”

“…Until recently, DCD was the only deceased organ donation practice in Japan. 3 European countries have been utilizing DCD organs for donation for more than 20 years, 4 and in the USA, DCD practices have been in place for at least the last decade, and even longer in a few regions. 5 In contrast, Canada abandoned DCD as soon as ''brain death'' was defined in 1968 and has been late to reintroduce DCD practices.…”
