Long-term carcinologic results of advanced esthesioneuroblastoma: a systematic review

Abstract: Surgical resection followed by radiotherapy can be considered like the optimal treatment modality for limited esthesioneuroblastoma. However, therapeutic management of locally advanced tumors remains a challenge. The aim of our study was to access and compare the oncologic results of the different treatment modalities in advanced esthesioneuroblastoma. We performed a systematic review using the Medline, and Cochrane database in accordance with PRISMA criteria and included all the cases of advanced esthesioneur… Show more

“…Sometimes, their removal may involve the resection of the anterior skull base owing to their attachment or its endocranial extension through the cribriform plate. Endoscopic endonasal surgery is considered as a safe and efficient approach for sinonasal cancer extirpation with a low morbidity and equivalent oncologic results compared to craniofacial resection . However, issues remain concerning ONB, because its prevalence is very low, its behavior is sometimes polymorphous, and its topography prevents creating margins and causes limitations owing to the difficulty of knowing whether the resection is complete or not.…”

“…In human patients with ONB, surgery followed by radiation therapy remains the 'gold standard' therapy. 2,3 However, prognosis varies greatly depending on both stage and grade. In human patients, the modified Kadish staging system categorises ONB as A (nasal passages only), B (nasal and paranasal sinuses), C (invasion of cribriform, skull base or intracranial cavity) or D (metastatic disease).…”

“…A small number of studies recommend single modality therapy with either radiation or surgery in Kadish A tumors; 33,34 however, the vast majority of studies suggest that combined modality therapy is most appropriate in the management of esthesioneuroblastoma as it has been reported to improve local tumor control, particularly in advanced stage and high-grade disease. 35,36 Adjuvant radiation can be used alone or concurrently with chemotherapy postoperatively as cisplatin-based chemotherapy is active against esthesioneuroblastoma even in advanced stage and high-grade disease. 22,37 Neoadjuvant treatment with chemotherapy and/or radiation may be of benefit in advanced disease; 38 however, further studies are necessary to determine its effectiveness.…”
