Long-term assessment of anorectal function after extensive resection of the internal anal sphincter for treatment of low-lying rectal cancer near the anus

Abstract: Abstract:Objectives: Intersphincteric resection (ISR) for low-lying rectal cancer (LRC) may induce major problems associated with anorectal function. In this study, we assessed the severity of ISR-induced impairment in anorectal function. Methods: In total, 45 patients followed up regularly !2 years after diverting ileostoma closure were eligible. The patients underwent ISR (n=35) or conventional coloanal anastomosis without resection of the internal anal sphincter (IAS) (n=10) for treatment of LRC from Januar… Show more

“…In this series, we were able to preserve the IAS completely in 18 patients (28.8%) using the TARD technique; consequently, this might minimize postoperative anorectal dysfunction in these patients. These results show again the significance of preserving the IAS for anorectal function after surgery [49] .…”

“…In addition to the clinical and oncological outcomes, the anorectal functional outcome was another essential indicator for the safety evaluation of ISR. The present studies showed that the excision of the internal anal sphincter had negative effects on short- and long-term anorectal function, and some patients even suffered from complete incontinence resulting in a conversion to a permanent colostomy ( 6 , 21 ). Kirwan's grade and Wexner score were applied to evaluate the defecatory function after ISR.…”

“…It can also damage the anal sphincter and promote fibrosis [ 25 , 26 ] directly. In addition, Shiokawa et al [ 81 ] has confirmed the negative effect of extensive internal sphincter resection on anal function. Huang et al [ 82 ] compared the anal function after LISR with OISR, and the result showed that the ratios of patients with good continence were 87.1% and 87.5%, respectively, during a mean follow-up of 52.0 months.…”
