Localization of insulinomas: Comparison of conventional arterial stimulation with venous sampling (ASVS) and superselective ASVS

Abstract: SS-ASVS is a useful method for detailed evaluation of overproduction of insulin from pancreatic insulinomas and their localization. When the pancreatic insulinoma is situated in the pancreatic body or tail, the localization is more accurately made by SS-ASVS than by conventional ASVS.

“…Somatostatin receptor scintigraphy (SRS) using radiolabeled octreotide is extremely sensitive in detecting gastrin-producing tumors, but is less sensitive in the visualization of insulinomas (14). Selective catheterization techniques can be applied to determine whether different pancreatic regions harbor endocrine active tumors (15), but the utilization of these methods can be limited by their invasive nature.…”

“…11,22,[33][34][35] SRS has become the primary diagnostic imaging technique for the identification and staging of malignant PETs in humans [9][10][11][12]14 and is considered more sensitive in detecting metastases than conventional US and CT. 11,13,14 Lesion detection with SRS depends on the accumulation of radioactivity, which is determined primarily by the size of the lesion and the density of somatostatin receptors. 36 The primary tumors detected in this study were relatively small (md-Control Յ 2 cm in 10/14) compared with malignant PETs such as pancreatic gastrinomas (2-3 cm).…”

“…Durante a realização do exame é necessário que a glicemia seja monitorada e que todo o material para corrigir hipoglicemia grave esteja preparado para utilização em caso de emergência. Embora a incidência de complicações seja baixa, pacientes recebendo digitálicos devem ser cuidadosamente observados, devido a possível ocorrência de arritmia (13,14) . Pode ocorrer também a indução de pancreatite pelo cálcio.…”
