Local and Metastatic Relapses in a Young Woman with Papillary Squamous Cell Carcinoma of the Uterine Cervix

Abstract: Papillary squamous cell carcinoma (PSCC) is a rare histological type of cervical carcinoma whose biological behavior has not been fully established. A 33-year-old woman with an exophytic cervical mass underwent radical hysterectomy and bilateral pelvic lymph node dissection. Histological examination of the tumor revealed numerous papillary fronds lined by atypical stratified squamous cells, resembling high-grade squamous intraepithelial lesions or urothelium. She was diagnosed with stage IB1 PSCC. Three months… Show more

“…Even if LEEP is performed before surgery, it is possible that a second surgery or supplementary radiotherapy and chemotherapy might be required, especially if the lesions are diagnosed as IA1 stage and precancerous by LEEP. However, direct extensive total hysterectomy without LEEP might be an overtreatment for 10% of the cases; hence, radical hysterectomy can be performed as a nonfertility-sparing approach with informed consent ( 15 , 16 ). If imaging results are negative, then cervical conization should be performed for precise treatment ( 17 ).…”

“…Although it is believed that this subtype shares the same risk factors and prognosis as SCC, it is more likely to metastasize and recur [14,15]. Even a few months after finishing treatment, there have been examples of early recurrence reported in the literature, which is indicative of a more aggressive course [15,16].…”

“…The incidence of PSCC is estimated to be 1.6% but given the number of unpublished and unrecognized cases it may be higher [15].…”

“…In terms of risk factors for onset and clinical behavior, PSCC is considered to be similar to classical SCC, with the potential for late local recurrences and metastasis [10,15]. There is currently no specific treatment for PSCC [15], and it should be treated as SCC according to the stage of the disease [9,16].…”
