Liver transplantation in Acute-on-Chronic liver failure: Timing of transplantation and selection of patient population

Abstract: Acute-on-Chronic liver failure (ACLF) is a clinical syndrome with high short-term mortality. Alcoholic ACLF is prevalent in European and American countries, while hepatitis B virus (HBV)-related ACLF is more common in the Asia-Pacific region. There is still a lack of a unified definition standard for ACLF, due to various etiologies and pathogeneses in different continents. Currently, liver transplantation (LT) is the most effective treatment for liver failure. However, the shortage of liver sources is still a … Show more

“…Transplant is the only ultimate standard therapy for severe ACLF that does not rely on liver regeneration for clinical improvement; 1-year post-transplant survival has been shown to be over 80% regardless of ACLF grade, and is better among transplant recipients compared to non-recipients[ 36 , 37 ]. However, patient selection is crucial given the narrow window for transplant[ 38 , 39 ]. Several studies have examined pre-transplant predictors to prognosticate post-transplant survival in ACLF[ 39 , 40 ].…”

“…PE is widely used in China and other Asia-Pacific countries. It can remove toxic substances from the bodies of patients within the appropriate timeframe and reduce liver inflammation [22]. At the same time, it also supplements plasma proteins and coagulation factors, which can not only reduce edema and bleeding in patients but also reduce the chance of infection and facilitate the repair and regeneration of liver cells.…”

“…In the literature, liver transplantation in ACLF has been studied in retrospective series with contrasting results. Indeed, post liver transplantation survival rates for ACLF-3 vary from 52 to 93% at 1 year [ 2 , 10 , 11 , 12 , 13 , 14 , 15 ]. Among the largest series published in the literature, the French multicenter study of Artru et al involving 73 patients transplanted with ACLF-3 showed interesting results [ 12 ].…”

“…Liver transplantation in ACLF has been reported in retrospective series and registries with contrasting results. Post-liver transplantation survival rates for ACLF-3 varied from 52 to 93% at 1 year [ 2 ]. These differences are probably related to differences in patient selection and centers experience in critically ill patients.…”
