Liver Preservation With Machine Perfusion and a Newly Developed Cell-Free Oxygen Carrier Solution Under Subnormothermic Conditions

Abstract: We describe a new preservation modality combining machine perfusion (MP) at subnormothermic conditions (21°C) with a new hemoglobin‐based oxygen carrier (HBOC) solution. MP (n = 6) was compared to cold static preservation (CSP; n = 6) in porcine orthotopic liver transplants after 9 h of cold ischemia and 5‐day follow‐up. Recipients' peripheral blood, serial liver biopsies, preservation solutions and bile specimens were collected before, during and after liver preservation. Clinical laboratorial and histologica… Show more

“…Biliary complications were significantly lower in the HMP group compared to the CS group [46]. Fontes et al recently reported fully oxygenated perfusion of liver grafts under sub-normothermic perfusion of liver grafts conditions with a new hemoglobin-based oxygen carrier solution [48]. In an animal model, they proved that this perfusion modality was associated with a downregulation of inflammatory products in liver tissue: liver grafts cleared lactate, produced urea, sustained gluconeogenesis, and produced bile with improved graft function.…”

“…Typically, perfusates based on concentrates of red blood cells, plasma, nutrients, cofactors and insulin, antibiotics, electrolytes and buffers are required, which makes normothermic machine perfusion a complex and expensive procedure [124]. To avoid the use of human blood products, the interest in extracellular oxygen carriers or oxygen-carrying plasma expanders has increased [131]. …”

“…34 However, as a result of ischemia during OLT, a sterile immune response is initiated, with elevated levels of proinflammatory cytokines (eg, interferon-γ and interleukins) and infiltration of inflammatory cells into the biliary epithelium, which can result in an upregulation of HLA class II antigens. 21,35,36 Ischemiareperfusion injury (IRI) of the biliary epithelium is also considered as one of the most important risk factors for NAS development. The effect of IRI is not limited to cholangiocytes only.…”
