Organ Preservation into the 2020s: The Era of Dynamic Intervention

Abstract: Organ preservation has been of major importance ever since transplantation developed into a global clinical activity. The relatively simple procedures were developed on a basic comprehension of low-temperature biology as related to organs outside the body. In the past decade, there has been a significant increase in knowledge of the sequelae of effects in preserved organs, and how dynamic intervention by perfusion can be used to mitigate injury and improve the quality of the donated organs. The present review … Show more

“…Lower temperatures have been shown to be advantageous for transportation of ovarian fragments to be used for several purposes in different species [ 40 , 57 , 83 , 84 ]. The present work shows that even after a 24 h cooling process before grafting, the percentages of normal and developing follicles, the vessel density/cm 2 (CD-31), and the proportion of apoptotic stromal cells labeled with caspase 3 were similar to those of transplanted fragments previously kept at room temperature for 30 min.…”

“…In the setting of prolonged ischemia time, low-potassium dextran has consistently demonstrated superior graft function in animal models. 13 In general, a flush with high volume and high flow has been shown to decrease the incidence of LIRI and improve posttransplant lung function. A single anterograde pulmonary artery (PA) flush is standard because of its technical ease, targeting a volume of 60 mL/kg and PA pressure between 10 and 15 mm Hg.…”

“…While vital organ preservation has been developed and applied over the past 60 years [reviewed by Petrenko et al (2019)], preservation of ovarian tissue is relatively new.…”
