Liposuction in cervical rejuvenation

Abstract: Superficial musculoaponeurotic system (SMAS) has represented a confusing anatomical structure because descriptions of it in classical treatises of anatomy are contradictory. Also, utilization of this system in facial rejuvenation also does not coincide with the true anatomical facts regarding the superficial musculo-aponeurosis. A macroscopic and histological study of the region was carried out in order to determine the areas of fat deposits and the distribution of the true cervical superficial aponeurosis, pa… Show more

“…Early healing irregularities may be seen but are usually fleeting. 7,10,16,17,18,20,21,22 Asymmetries, dimples, or bulges that persist beyond 6 months postoperatively are often amenable to additional liposuction, subcutaneous steroid injection, or localized fat injection. 12 As noted above, SAL demonstrates numerous advantages over techniques designed for direct fat excision.…”

“…From here on if we wanted to work on the lower half of the face and neck we had to go on to a more superficial plane and make a dissection in the SMAS [7]. The lifting and pinning down of this frontoparietomalar flap is done by attaching the superficial temporal aponeurosis to the deeper one in the lateral and upper (frontoparietal) regions--as if we were going to tighten the tail of the eyebrows--and then continuing into the central area, though by now with less tension.…”

“…The media temporal vein can also be seen crossing this space. It is important to leave the frontal branches [7] (Fig. 6), which cross the surface of the zygomatic arch, undamaged by avoiding putting any pressure on them from the periosteum or dissecting them when approaching this periorbital-zygomatic area.…”

“…Liposuction provides many distinct advantages when combined with rhytidectomy, including reduced blunting of the cervicomental angle and volumetrically reduced jowl volume . The main challenge in combining liposuction with facelifting remains not only control of adipolysis, but also the degree of overlying skin contraction through neocollagenization .…”

“…[12][13][14]15 According to the American Society for Aesthetic Plastic Surgery, in 2011, liposuction was the most popular cosmetic surgical procedure having over 325 000 procedures. 3 Liposuction provides many distinct advantages when combined with rhytidectomy, 4 including reduced blunting of the cervicomental angle 5 and volumetrically reduced jowl volume. 6 The main challenge in combining liposuction with facelifting remains not only control of adipolysis, but also the degree of overlying skin contraction through neocollagenization.…”
