Linear relationship between lymphocyte counts in peripheral blood and buffy coat collected during extracorporeal photopheresis

Abstract: The preprocedural peripheral lymphocyte count can predict the number of lymphocytes within the buffy coat collected during ECP, which may justify the use of peripheral lymphocyte count as a surrogate for the cell dose treated per procedure. Peripheral monocyte counts may serve as an alternative. CELLEX is more efficient in collecting lymphocytes and monocytes than UVAR XTS under conditions tested.

“…Adverse events were closely monitored throughout the procedure. Buffy coat was sampled at the end of the collection phase of the procedure as described previously . Cell counts with differentials were performed on these samples, and the LFE was compared between the prospective cohort and two historical control groups consisting of GVHD patients with haematocrits of 29% and higher treated using either CELLEX or UVAR XTS photopheresis system (Mallinckrodt Pharmaceuticals, formerly Therakos Inc.).…”

“…There is no consensus on how to conveniently track the cell dose of ECP therapy without accessing the buffy coat for each ECP procedure. We reported previously a linear relationship between lymphocyte counts in the preprocedural peripheral blood and the buffy coat collected during ECP in patients with GVHD or chronic lung allograft rejection . Based on the linear relationship, the lymphocyte fold enrichment (LFE) provides a valid quantification of the collection efficiency, which can be compared across different instruments used for ECP treatment.…”

“…Therefore, ECP is more challenging in patients with suboptimal vein conditions. In a cohort of 52 ECP treated patients with GvHD or lung transplant rejection the CVC rate was 44.2% .…”

“…The additional fluid is expected with the MBP given the larger return volume that contains RBCs, anticoagulant, and saline. Comparison of the concentration of white blood cell (WBC) fractions as defined by WBC‐fold enrichment, adverse events, and system alarms were considered clinically similar between the two blood prime procedures.…”
