The ‘Intermittent Hypoxia and Caffeine in Infants Born Preterm (ICAF)’ study (NCT03321734) uses an extemporaneously compounded enteral caffeine base solution for its study drug. The primary aim of this report is to determine the stability of this specific enteral caffeine base preparation stored for up to 6 months and assess optimal storage temperature. To analyse stability, caffeine solutions were prepared and stored at 4°C and 25°C (room temperature). The caffeine concentrations were analysed over time using high‐performance liquid chromatography (HPLC). To confirm the accuracy of compounded caffeine concentrations, study drug samples from three research pharmacies were analysed. Stability results showed that caffeine concentrations are within 5% of the expected concentration when stored for up to 6 months at room temperature. Our results also show that accurate caffeine concentrations were achieved by multiple research pharmacies.

At the hospital, the pharmacist is constantly challenged to prepare extemporaneous solutions from tablets, capsules or drug powder for patients unable to swallow, such as pediatric, elderly and patients that use nasoenteric and nasogastric tubes. The preparation of extemporaneous solutions from capsules, tablets and drug powder requires stability studies analysis. This article is a bibliographic review of preparation of extemporaneous oral liquid from solid oral dosage forms used in clinical practice. The selected articles contain all the information regarding manipulation techniques, pharmaceutical excipients, packaging, storage conditions and results of stability studies above 90% performed by HPLC analysis. In addition, a situational analysis of the strategies for the preparation of the extemporaneous solution was described to help the manipulator in the decision. The preparation of extemporaneous solution from solid oral dosage forms is based on information from official compendium or scientific literature, to ensure safe and effective manipulated medicine.

BackgroundLevofloxacin is a broad‐spectrum antibiotic of the fluoroquinolone class used in the paediatric population to treat resistant bacterial infections. Because no commercial option exists, a stable liquid formulation of this antibiotic made with accessible vehicles would be useful.AimThe aim of this study was to develop and assess the stability of levofloxacin suspensions made from commercially available tablets and three different accessible vehicles.MethodsLevofloxacin suspensions (50 mg/mL) were compounded using two different commercially available tablets (from Apotex; Toronto, ON, Canada and Sandoz; Boucherville, QC, Canada) and three different vehicles (Ora‐Blend, Oral‐Mix; Medisca, Saint‐Laurent, QC, Canada and Simple Syrup (SS); Laboratoire Atlas Inc., Anjou, QC, Canada). Suitable suspensions in Ora‐Blend and Oral‐Mix were stored at room temperature (25°C) as well as under refrigeration (5°C) in amber plastic bottles for 90 days. Suspensions made with SS were only stored at 25°C because of sugar crystallisation at 5°C. Suspension concentrations were assessed by HPLC.ResultsSuspensions using Apotex levofloxacin tablets produced unsuitable high‐foaming, phase‐separated, non‐homogeneous suspension products in all vehicles tested that were therefore not tested for stability. Levofloxacin suspensions prepared using Sandoz tablets remained chemically and physically stable for 3 months, with >96% recovery according to United States Pharmacopeia criteria (90%–110%) in all three vehicles.ConclusionThe results show that the choice of generic levofloxacin tablets is important when preparing homogeneous suspensions. All three vehicles could be used to prepare levofloxacin suspensions using Sandoz tablets that were physically and chemically stable for 3 months.
