Lichen planus of the nail matrix with predominant plasma cell infiltrate

Abstract: Lichen planus (LP) is an inflammatory dermatitis of idiopathic origin that can involve the skin, mucous membranes, hair and nails. Histologically, LP is characterized by compact orthokeratosis, wedge-shaped hypergranulosis, irregular acanthosis, damage to the basal cell layer and a band-like inflammatory infiltrate in the upper dermis. Lymphocytes are the predominant cells making up the infiltrate, along with a few macrophages, eosinophils and plasma cells. In addition, melanophages are often found in the uppe… Show more

“…Epithelial hyperkeratosis was observed in 66% of our patients, being more predominant in the reticular form. This increased thickness of the corneal layer has also been described in several other studies (15,16). As regards acanthosis, our data is consistent with that found in the literature (15,16), with percentages of approximately 48%.…”

As regards acanthosis, our data is consistent with that found in the literature (15,16), with percentages of approximately 48%. The presence of serrated ridges is another one of the histological findings of oral lichen planus observed in 30% of our sample and described by other authors (15).…”

“…Lichen planus has an unknown etiology, but it seems that it is an immunological process with a delayed hypersensitivity reaction, in microscopic views (3). Due to having symptoms of irritation and pain, especially in erosive and atrophic form, and the risk of malignancy, the treatment of these lesions is of the utmost importance (2).…”
