In 2019/2020, the emergence of coronavirus disease 2019 (COVID-19) resulted in rapid increases in infection rates as well as patient mortality. Treatment options addressing COVID-19 included drug repurposing, investigational therapies such as remdesivir, and vaccine development. Combination therapy based on drug repurposing is among the most widely pursued of these efforts. Multi-drug regimens are traditionally designed by selecting drugs based on their mechanism of action. This is followed by dose-finding to achieve drug synergy. This approach is widely-used for drug development and repurposing. Realizing synergistic combinations, however, is a substantially different outcome compared to globally optimizing combination therapy, which realizes the best possible treatment outcome by a set of candidate therapies and doses toward a disease indication. To address this challenge, the results of Project IDentif.AI (Identifying Infectious Disease Combination Therapy with Artificial Intelligence) are reported. An AI-based platform is used to interrogate a massive 12 drug/dose parameter space, rapidly identifying actionable combination therapies that optimally inhibit A549 lung cell infection by vesicular stomatitis virus within three days of project start. Importantly, a sevenfold difference in efficacy is observed between the top-ranked combination being optimally and sub-optimally dosed, demonstrating the critical importance of ideal drug and dose identification. This platform is disease indication and disease mechanism-agnostic, and potentially applicable to the systematic N-of-1 and population-wide design of highly efficacious and tolerable clinical regimens. This work also discusses key factors ranging from healthcare economics to global health policy that may serve to drive the broader deployment of this platform to address COVID-19 and future pandemics.

Rapid and accurate antimicrobial susceptibility testing (AST) of bacterial infections can improve patients’ outcome. In this study, 29 Klebsiella pneumoniae strains isolated from sputum specimens from ICU patients at the Jiading Central Hospital were compared using D2O-Raman AST and traditional AST (Vitek 2). The analysis time was 4–4.5 h for a complete D2O-Raman AST result. Compared with the Vitek 2, the results of D2O-Raman spectroscopy showed good agreement for susceptibility and resistance (S/R), with a concordance of 89%. Most discrepant results were observed for intermediate and susceptible categories, as 10% of isolates labeled as intermediate and susceptible by Vitek 2 were almost all categorized as resistant by D2O-Raman spectroscopy. Moreover, discriminant factor analysis of our D2O-Raman results led to successfully discriminating the type of resistance. In addition, we evaluated the behavior of clinicians in adjusting antibiotic therapies according to routine AST results. For 9 out of 10 patients, antibiotic therapy was adjusted after the reporting of AST results. With some improvements, D2O-Raman spectroscopy provides an accurate and rapid preliminary AST result and may have at least a potential clinical benefit for patients.

<p>Pneumonia merupakan penyakit infeksi yang masih menjadi permasalahan di dunia turut menyumbang angka mortalitas dan morbiditas. Penyakit infeksi dapat disebabkan oleh bakteri, virus, dan jamur. Pengobatan utama penyakit infeksi akibat bakteri adalah penggunaan antibiotik. Tingginya intensitas penggunaan antibiotik yang dapat menimbulkan berbagai permasalahan dan ancaman bagi kesehatan perlu untuk dikaji. Penelitian yang dilakukan merupakan penelitian observasional untuk mengetahui profil pasien dan penggunaan obat pasien pneumonia. Sampel dalam penelitian ini diambil menggunakan teknik <em>total sampling.</em> Data diambil secara retrospektif dari rekam medis dan dianalisis secara deskriptif. Hasil penelitian menunjukkan bahwa sebagian besar pasien pneumonia adalah pasien usia >65 tahun berjenis kelamin laki-laki dengan lama perawatan terbanyak 1-7 hari (81,8%), derajat keparahan terbanyak “Ringan” (45,5%), dan penyakit komorbid terbanyak kardiovaskuler (50,9%). Terapi antibiotik tunggal terbanyak adalah levofloxacin (38,2%), evaluasi penggunaan obat yang telah sesuai sebanyak 14,5%, dan potensi interaksi obat terbanyak pada levofloxacin dengan methylprednisolon (26,4%).</p>
