BackgroundSmart devices and mobile applications are now an integral part of all aspects of everyday life. They are particularly numerous in the field of health, contributing to the movement called ehealth. What is the potential role of these devices as prevention supports? The purpose of this article is to provide an exploratory analysis of the use, efficacy and contribution to conventional prevention strategies.MethodsTo address this issue, we conducted a scoping-review on the basis of 105 publications from the fields of medicine and human sciences.ResultsThree dimensions of the use of smart devices in the field of health were identified: 1/a quantification tool allowing the users to measure their activities; 2/a tool of self-positioning in the community; 3/an interface between the medical world and the population, modifying the hierarchy of knowledge. However, few published studies have investigated the determinants of the efficacy of these devices and their impact on individual behaviours and professional health practices.ConclusionBased on the hypothesis of possible integration of these devices in prevention policies, it would be interesting to investigate two research issues: how and under what psycho-socio-environmental conditions can smart devices contribute to the adoption of positive health behaviours? To what degree does the use of smart devices modify the health care professional-patient relationship? Finding answers to these questions could help to define the real place of these devices in prevention strategies by determining their complementarity with respect to other prevention strategies, and the conditions of their efficacy on behaviours and inequalities.

International audienceIntroduction: The durability of telehealth projects is too often questioned by the lack of prior development of sustainable business models allowing the finding of the equilibrium of the funding for the deployment and for the operational phases. The existing medico-economic and multidisciplinary methods do not address this problem; hence a new one is needed.Materials and Methods: The complex system design tools used in the field of Industrial Engineering are used as a base for this new method. The holistic approach adopted represents an opportunity to develop sustainable business models taking into account needs and viewpoints of each stakeholder of a telehealth project.Results: The proposed method aims at creating various scenarios of business models and offering a clear vision on how the proposed changes will impact each of the stakeholders involved as well as the overall system. An example of how this can be applied is detailed.Discussion and Conclusions: The presented method allows the design of economic value sharing scenarios between all the stakeholders of a telehealth project. Its application to a particular one can act as a basis for discussions during negotiations between stakeholders. It can also support the assessment of the overall project’s sustainability.Introduction : La pérennité des projets de télémédecine est trop souvent remise en cause par l'absence de développement préalable de modèles économiques durables qui permettraient d'élaborer de nouvelles organisations à même d'équilibrer le financement de leur déploiement et de leur fonctionnement. Les méthodes d'évaluation médico-économiques et les méthodes multicritères existantes ne permettant pas de répondre à cette problématique, une nouvelle méthode est donc nécessaire. Matériels et Méthodes : Les outils d’analyse des systèmes complexes utilisés dans le domaine du Génie Industriel sont mobilisés pour créer cette méthode. La perspective holiste adoptée est l’occasion d’élaborer des modèles économiques plus durables pour chacun des acteurs d’un projet de télémédecine.Résultats : La méthode proposée permet de créer des scenarii variés de modèles économiques et offre une vision précise de ce que l’évolution projetée aura comme impact sur chacun des acteurs impliqués et sur le système. Un exemple d’application est détaillé.Discussion et Conclusions : La méthode présentée permet de concevoir des scenarii de partage de la valeur économique entre les acteurs d'un projet de télémédecine. Son application à un projet particulier permet de servir de support de discussion lors des négociation

New technologies including digital health and robotics are driving the evolution of healthcare. At the same time, healthcare systems are transitioning from a multiprofessional model approach of healthcare delivery to an interprofessional model. The concurrence of these two trends may represent an opportunity for leaders in healthcare because both require renegotiation of the complex division of work and enhanced interdependency. This review examines how the introduction of new technologies alters the role boundaries of occupations and interdependencies among health occupations. Based on a scoping review of ethnographic studies of technology implementation in a variety of contexts (from primary care to operating room) and of diverse technologies (from health informatics systems to robotics), we develop the concept of role reconfiguration to capture simultaneous adjustments of multiple, interdependent roles during technological change. Ethnographic and qualitative studies provide rich, detailed accounts of what people actually do and how their work and role is changed (or not) when a new technology arrives. Through a synthesis of these studies, we develop a typology of four types of role reconfiguration: negotiation, clarification, enlargement and restriction. We discuss leadership challenges in managing role reconfiguration and formulate four leadership priorities. We suggest that leaders: redesign roles proactively, paying attention to interdependencies; offer opportunities for collective learning about new technologies; ensure that knowledge of new technologies is distributed across roles and prepare to address resistance.
