Lenient versus Strict Rate Control in Patients with Atrial Fibrillation

Abstract: In patients with permanent atrial fibrillation, lenient rate control is as effective as strict rate control and is easier to achieve. (ClinicalTrials.gov number, NCT00392613.)

“…560,561 Similar results were found in a pooled analysis of the AFFIRM (Atrial Fibrillation Follow-up Investigation of Rhythm Management) and RACE trials (1091 participants), albeit with smaller heart rate differences and without randomization. 562 It is worthwhile to note that many 'adequately rate-controlled' patients (resting heart rate 60-100 b.p.m.)…”

“…docelowej częstości rytmu komór < 110/min. Nie stwierdzono różnicy łącznej częstości występowania incydentów klinicznych (14,9% w grupie ścisłej kontroli częstości rytmu komór vs. 12,9% w grupie mniej rygorystycznej kontroli częstości rytmu komór) [560], klasy NYHA ani częstości hospitalizacji [560,561]. Podobne wyniki uzyskano w łącznej analizie badań Atrial Fibrillation Follow--up Investigation of Rhythm Management (AFFIRM) i RACE (1091 uczestników), choć w tej analizie różnica częstości rytmu komór była mniejsza i nie prowadzono randomizacji do grup o różnej docelowej częstości rytmu [562].…”

“…3,4) Accordingly, rate control has been adopted as a front-line therapy in the management of AF. 5) AF, however, is also more strongly associated with poor mortality and morbidity due to congestive heart failure and stroke, compared to sinus rhythm (SR). The mortality risk gets much worse when AF develops in patients with heart failure.…”
