Leishmaniasis in the genital area

Abstract: Two patients from the gold mines of Bolivar State, Venezuela, presenting cutaneous leishmaniasis in the genital region, an unusual location, are described. The first patient showed an ulcerated lesion of the glans penis. Leishmanin skin test was positive. A biopsy specimen revealed a granulomatous infiltrate containing Leishmania parasites. In the second patient, Leishmanin skin test was positive, HIV and VDRL were negative. Leishmania parasites were present in a biopsy of an ulcerated lesion in the scrotum, w… Show more

“…In addition to dissemination and lesions affecting the mucosa, unusual manifestations comprised infiltrated or erythematous plaques, extensive ulcers and unusual locations such as the lips, but more strikingly in the genital areas of a large proportion of patients (27%), which is not common in either tegumentary leishmaniasis alone or with concomitant HIV infection as known so far. [27][28][29] The lesions in the genital areas were most probably due to leishmaniasis, as they improved after specific treatment for leishmaniasis, and in one case Leishmania antigens were detected in the lesion itself. To the best of our knowledge this is the first report of genital involvement by leishmaniasis in an HIV-infected patient from the New World.…”

“…11 However, the amount of the inoculum was not stated in the paper, and mating was conducted with a chronically infected male. Therefore, the contribution of unknown pre-disposing factors, such as the concentration of the inoculum and factors related to sexual transmission (a situation that has been described based on the appearance of genital lesions in humans 22,23 and animals 2 harboring the visceral species), could not be excluded. For cutaneous Leishmania species, such as L. mexicana, and L. tropica, the possibility of vertical transmission has not been considered, because it is typically thought that the parasite is limited to skin infections.…”
